The population health nurse will coordinate and administer the annual Health Partners of New Hampshire Physician Hospital Organization (PHO) quality programs. The role also acts as a liaison helping to facilitate population health initiatives within primary care and specialty practices.

  • Experience Minimum Required

  • Ability to work in a small team environment as well as collaboratively with multiple outpatient practice staff and providers.

  • Must be able to effectively identify opportunities for improvement in clinical and operational performance and work independently on-site with the practice to achieve performance goals.

  • 3 years nursing experience in a hospital or outpatient setting

  • Experience Preferred/Desired

  • RN with 5 years in healthcare environment.

  • Experience in care coordination or quality improvement experience, including data management.

  • Education Minimum Required

  • RN with strong clinical background in outpatient primary and/or specialty care.

  • Will consider acute care experience if candidate also has process improvement experience.

  • Bachelors Degree in Nursing

  • Education Preferred/Desired

  • Masters Degree in Nursing or health related field (e.g. MHA, MPH ).

  • MBA will also be considered if candidate has required healthcare experience.

  • Training Minimum Required

  • Recent experience in performance improvement as a team member or facilitator.

  • Training Preferred/Desired

  • Training in quality/performance improvement strongly desired.

  • Credentialed in Quality Improvement, Lean/Six Sigma or equivalents.

  • Must have strong skills in data interpretation and translation.

  • Special Skills Minimum Required

  • Ability to use performance improvement tools to facilitate sustainable improvements in outpatient clinical teams.

  • Ability to interact well orally and through written correspondence with all aspects of the organization.

  • Strong skills in Microsoft Excel and PowerPoint.

  • Must be very comfortable working with large data sets.

  • Special Skills Preferred/Desired

  • Must be comfortable working with large data sets with multiple variables.

  • Microsoft Access or SQL is a plus.

  • Registered Nurse in New Hampshire

  • Licensure and/or Certifications Required

  • Licensure and/or Certifications Preferred/Desired

  • Clinical nurse specialist
