As a Population Health Director (PHD), you will design and implement innovative population health initiatives and programs to improve quality and cost of care, resulting in overall improved health outcomes for our patients. PHDs are cross-team leaders who work side-by-side with providers, care teams, field leaders, and Treehouse (HQ-based) teammates to make lasting impacts.

Being a PHD is an important step toward General Management preparation at Oak Street Health. You'll not only have the opportunity to make a measurable impact on our business in the first 6 months, but as a change agent and team player, you will be responsible right out of the gate for the success of programs that touch our patients lives.

PHDs will have the opportunity to become a Regional Vice President, with $30M+ P&L ownership within 12 to 24 months.

We currently operate centers across Illinois, Indiana, Michigan, Ohio, Pennsylvania, Rhode Island and are opening centers in North Carolina and Tennessee with plans to open more centers in multiple states per year for the foreseeable future.

What are we looking for?

  • Recent MBA or other advanced degree graduates

  • Leadership experience required, healthcare experience preferred

  • Supportive attitude toward our patient population of older adults

  • Outstanding communication skills

  • Strong analytical and financial competencies

  • Incredible levels of tenacity, empathy and willingness to roll up your sleeves

  • Permanent US work authorization required

  • To note, this role/program will begin in August 2020

This position reports to the Senior Director of Leadership and Annual Giving and works closely and collaboratively with team across the office, with a special focus on the Prospect Management and Major Gifts teams. The individual who holds this position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures. Position Responsibilities: * Achieve annual metrics for visits and solicitations as well as hand-offs to the Major Gift team for assignment as Major Gift prospects. * Support the attainment of Leadership and Annual Giving team goals with a focus on the number of gift club members, retention and upgrade rates, and total dollars raised. * Maintain an aggressive visit schedule to assigned prospects to meet metric goals. * Identify, cultivate and solicit prospects capable of making gifts of $10,000-$50,000 with a heavy emphasis on retaining/upgrading existing donors and reengaging past donors who are not assigned to other staff. * Qualify prospects for potential major gift capacity and interest. * Provide appropriate background on prospects to be assigned to Major Gift portfolios. * Serve as liaison between the Annual Giving and Major Gifts teams. * Collaborate with team members to develop integrated strategies and tactics for the leadership annual giving segment, leveraging direct mail appeals, communications, events and stewardship tools to achieve goals. * Adhere effectively and appropriately to prospect management guidelines by completing Advance system updates in a timely and accurate manner. * Track and evaluate visit activities, report on progress and contribute to goal setting. * Other relevant duties as required. Position Qualifications Include: * Bachelor's degree required. * Five or more years of increasingly responsible positions in development or a related field, preferably in a health care or higher education setting. * Excellent oral and written communication skills, analytic skills and interpersonal skills. * Initiative, sound judgment and an ability to work collaboratively to move programs and prospects forward. Preferred Job Qualifications: Experience directly engaging constituents for fundraising purposes preferred, or experience in sales or related field.
