Overview

Reliant Medical Group is seeking a Population Health Coordinator to join our exciting and innovated organization!

Position: Full-time 38.75 hours per week

Hours: Monday through Friday 8:00 AM-4:30 PM occasional later hours until 5:00 PM

No Weekends, No Holidays

Department: Care Management

This position requires occasional travel to our other Reliant Medical Groups sites.

Responsibilities

Responsible for providing data coordination, and outreach for patient populations with chronic illnesses such as diabetes, hypertension or asthma as well as preventive health screenings.

Qualifications

  • High School diploma with additional specialized training preferred.

  • Knowledge and understanding of medical terminology strongly preferred.

  • Excellent organizational and interpersonal skills. Ability to work effectively with all levels of personnel.

  • Ability to collaborate closely managers and other staff.

  • Effective presentation skills.

  • Excellent individual problem solving skills and flexibility.

  • Must be able to work independently and be self-directed.

  • Proficiency with electronic medical record systems preferred, Microsoft Office: Excel, Word, Access, and PowerPoint required.

  • Proficient with data manipulation including the ability to develop spreadsheets, build data tables and extract data.

Additional software knowledge and skillset using Adobe Illustrator, Photoshop and CorelDraw skills required to assist MassCor Operations, Marketing & Sales as well as customer design work. Individual should be self-motivated with a high level of creativity and concept ideas. Must have the ability to develop rudimentary to complex CAD design, in addition to providing engineering plans to meet specifications. Candidate must be willing to work in multiple locations; position will require local travel to various customer locations to obtain required quoting specifications. Assist with installation and system configurations both software and hardware applications. Ability to assist and identify software and hardware concerns, within MassCor Industries operations, apply appropriate corrective measures. Provide daily server and system network backup tasks, conduct tests on systems equipment and software programs, test and assess new technology, assist with training staff on equipment. Prepare reference material for users by drafting operation instructions. Perform all other duties as required. Preferred Qualifications: Proficient experience in CAD design, Photoshop, Adobe Illustrator and CorelDraw with strong artistic, creative thinking, specification understanding and problem solving skills. Understanding of computer networks, servers and system management required. Ability to balance multiple competing priorities and deadlines, excellent organizational skills, as well as the ability to track deliver on multiple projects/deadlines tasks. Strong communication skills, should be self-motivated able to work independently or as part of a team. Ability to understand and follow instructions precisely. This is a Civil Service position.
