A Brief Overview

CAREER LADDER SUMMARY: The Population Health Coordinator functions as a member of a centralized team supporting ambulatory departments and the organization as a whole in achieving population health goals, improving patient experience and improving care coordination. The position performs routine outreach functions utilizing specialized skills, knowledge, tools and security access. Coordinators will regularly engage with patients and providers. As needed, the coordinators will connect patients with resources, schedule and coordinate care. Coordinators will routinely coordinate with different departments on behalf of patients. Coordinators will also routinely engage in activities related to data abstraction, workbench reporting, outreach preparation and front-line staff training. This role will require a high degree of comfort interacting with both patients as well as IT and reporting tools. Population Health Coordinators have three levels that are distinguished from each other primarily by: • The complexity of assigned duties and responsibilities. • Knowledge level of required clinical, reporting and population health skills • Required level of business communications skills. • Independence of action (degree of supervision received). JOB SUMMARY: Entry-level population health position with prior clinical experience as MA or LVN. Acquired job skills, policies, and procedures to complete substantive assignments/ tasks of moderate scope and complexity; exercises judgment within defined guidelines and practices to determine appropriate action. Performs work as assigned to support ambulatory clinics: • Chart Abstraction. • Preparations for outreach. • Responding to patient messages. • Patient Scheduling. • Patient phone calls with direction and support of other team members. • Manual data collection as needed to support projects.

  • Assignments are designed to provide training and practical experience that develops the incumbent's ability to apply principles, methodologies, and procedures.

  • Decisions are limited to specific, task-related activities.

  • Requires the manager's or lead's review of the work performed, while in progress and at its completion, for accuracy, completeness, and conformance with detailed instructions.

  • Work is primarily with existing, stable processes and procedures.

  • Customer Support/Interaction:

  • Limited client interaction.

Education Qualifications

  • High School diploma or GED equivalent.

  • Medical Assistant certificate/diploma from an approved school/institution or equivalent documented training.

  • OR

  • Current active license as Vocational Nurse in California.

Experience Qualifications

  • One (1) - two (2) years of clinical experience.
  • AND
  • Experience in at least 1 area below: • Outreach coordination. • Staff training. • Data collection/analysis. • Performance Improvement/Quality improvement.

Required Knowledge, Skills and Abilities

  • Beginning to Proficient in the MS Office Suite applications and Scheduling System.

  • Requires full knowledge and understanding of the theories, concepts, principles and practices of health care.

  • Will need to quickly acquire basic knowledge regarding population health, measurement systems, workbench reporting and excel.

  • Learns to apply principles, theories, and concepts through work assignments.

Preferred Knowledge, Skills and Abilities

  • Intermediate to Advanced proficiency in:

  • Database.

Base Pay Scale: Generally starting at $35.64 - $46.32 per hour

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
