This Population Health Assistant will be responsible for:
Working closely with our clinical and administrative teams to address the population health needs of our primary care patient population, with a particular focus on the newborn and asthma populations.
Preparing reports and records on population activities, progress, status or other special reports for supervisor.
Drafting written communication and promotional literature as part of the population management outreach efforts.
Assisting in planning workshops, meetings or conferences; coordinating logistics, scheduling and participant communications.
Interacting and maintaining liaison with staff, faculty and outside/community agencies in facilitating program objectives.
Coordinating the efforts of the performance improvement initiatives. Working with clinicians, administration and other facilities in order to coordinate collection of data for each population, tracking and trending this information in order to create interventions.
Providing administrative support for development of educational tools and training materials for quality improvement projects and data management related to assigned population.
Providing positive and effective customer service and contributing to teamwork within and between departments, divisions, programs and other organizations.
To qualify, you must have:
The knowledge of theories, principles, and concepts typically acquired through completion of Associates degree in quality management or other related field, such as in Nursing, Human Services or Business.
A minimum of one year experience in healthcare or a business setting.
Computer literacy including word processing, report preparation, and healthcare data management.
Excellent organizational, oral and written communication; the ability to communicate orally and in writing to a wide variety of audiences both formal and informally.
Good visual, verbal and listening skills, and analytical and problem-solving skills; as well as intense concentration and attention.
The ability to effectively communicate with all levels of personnel.
Basic knowledge of medical, mathematical and technical principles typically acquired through completion of associate's degree in the health sciences or related field.
Basic data management software (e.g. SPSS, MS,l).
Intermediate Excel knowledge.
Critical attention to detail and accuracy.
Knowledge of process mapping tools and Project Management principals.
