Helps develop and drive through the influential.

Collects, analyzes, and interprets data and presents actionable analytics in support of population health initiatives and the development of clinical and operational improvement and efficiency strategies under the general supervision of the Vice President for Community and Population Health.

Responsibilities Principal Duties:

  • Develops and maintains strong partnerships with major stakeholders, to ensure population health programs within the organization, address improvements in health and well-being of the community.

  • Develops and implements analysis and reporting programs for the organization and/or other population health management activities by ensuring data aggregation, measurement, analysis, and reporting to support improvement and redesign.

  • Evaluates the landscape of clinical transformation ("volume to value"), integration, quality measurement and reporting, and develops tactics to redesign processes to improve outcomes, lower costs, and utilize data effectively across these efforts.

  • Plays a key role in the selection and implementation of software solutions related to Population Health Management. Trains end users and clinical staff on available tools and management options.

  • Utilizes advanced querying techniques with thorough knowledge and understanding of data warehouse concepts, data cubes, and content. Works with advanced graphic tools to provide clear and insightful views of performance.

  • Creates and maintains up to date work flow and reporting documentation. Also acts a liaison between clinical users and IT management to maximize and support appropriate use application(s).

  • Designs and manages relational databases and data sets used for Population Health Management.


Experience (years): Required: 1-2 years

Experience with quality improvement programs, integration and informatics related to planning and population health management and involving:

Knowledge of NCQA recognition programs (PCMH, PCSP, DRP, HSRP), Meaningful Use PQRS/MACRA, Maryland CRISP HIE tools, patient engagement surveys and EMR based Reporting. Possession of a pragmatic, applied orientation to clinical and operational quality improvement and a solid grounding in health services/outcomes research and population health. Skill in interacting and communicating with physicians, nurses, finance experts, planning team and IT staff, including business analysts in other departments associated with analytics, planning and reporting.

Specialists will assist in managing community outreach projects including communications, special events planning and other related activities. Principal Responsibilities and Tasks The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. Plans, implements and monitors ongoing community health programs in support of the mission, values, goals and objectives of the system and local site as directed by manager or director. 2. Provides assistance with writing and/or purchasing collateral material to support community health initiatives. 3. Represents local site and/or UMMS at a variety of local health coalitions or with external community partners. 4. Works on planning and development of projects with a variety of internal and external partners. 5. Assists in the development and implementation of community health programs for targeted community population. 6. Demonstrates ability to manage, develop, plan and deliver projects with minimal direction while meeting deadlines. 7. Supports department leaders in other projects as needed including internal and external events. 8. Communicates project updates and plans with key stakeholders both internally and externally. Manages deadlines and multiple projects simultaneously, and completes work quickly and with high attention to detail. 9. Develops effective working relationships and maintains good communications externally with community partners and internally across the organization and system. 10. Identifies and participates in events, hosted by community partners to promote the image and reputation of the local site and UMMS. 11. Develops and maintains detailed reports of analytical results that track events and initiatives either using CBSA and/or other programs. 12. Performs other related duties as assigned or requested. What You Need to Be Successful: Education and Experience 1. Bachelor's Degree preferred or at least 1-2 years experience and training in a health related field – i.e. health education, nutrition, exercise science. 2. Two (2) to five (5) years of related or internship experience preferred. 3. Current CPR experience and/or Emergency First Aid training preferred.
