Overview

Helps develop and drive through the influential.

Collects, analyzes, and interprets data and presents actionable analytics in support of population health initiatives and the development of clinical and operational improvement and efficiency strategies under the general supervision of the Vice President for Community and Population Health.

Responsibilities Principal Duties:

  • Develops and maintains strong partnerships with major stakeholders, to ensure population health programs within the organization, address improvements in health and well-being of the community.

  • Develops and implements analysis and reporting programs for the organization and/or other population health management activities by ensuring data aggregation, measurement, analysis, and reporting to support improvement and redesign.

  • Evaluates the landscape of clinical transformation ("volume to value"), integration, quality measurement and reporting, and develops tactics to redesign processes to improve outcomes, lower costs, and utilize data effectively across these efforts.

  • Plays a key role in the selection and implementation of software solutions related to Population Health Management. Trains end users and clinical staff on available tools and management options.

  • Utilizes advanced querying techniques with thorough knowledge and understanding of data warehouse concepts, data cubes, and content. Works with advanced graphic tools to provide clear and insightful views of performance.

  • Creates and maintains up to date work flow and reporting documentation. Also acts a liaison between clinical users and IT management to maximize and support appropriate use application(s).

  • Designs and manages relational databases and data sets used for Population Health Management.

Qualifications

Experience (years): Required: 1-2 years

Experience with quality improvement programs, integration and informatics related to planning and population health management and involving:

Knowledge of NCQA recognition programs (PCMH, PCSP, DRP, HSRP), Meaningful Use PQRS/MACRA, Maryland CRISP HIE tools, patient engagement surveys and EMR based Reporting. Possession of a pragmatic, applied orientation to clinical and operational quality improvement and a solid grounding in health services/outcomes research and population health. Skill in interacting and communicating with physicians, nurses, finance experts, planning team and IT staff, including business analysts in other departments associated with analytics, planning and reporting.

In this role, the Director will oversee community outreach and benefit programs; including policies, community health needs assessments, program planning, development, and evaluation, management of certain outreach programs, and adherence to regulatory requirements and reporting for both organizations. The Director will have a dual reporting responsibility to the assigned affiliate leader and the Sr. VP, Government, Regulatory Affairs and Community Health. Principal Responsibilities and Tasks The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified. 1. Assesses the health needs of the communities served by UMMS affiliate through collaboration with community, government, and other agencies to conduct formal needs assessments focusing on vulnerable and underserved populations. 2. Reviews current local and national trends related to public health and community health improvement and emerging health issues for program planning and communication to the community. 3. Develops organization-wide specific community health priorities and implementation plans. Plans, develops, and evaluates appropriate community health programs based on the identified community needs. 4. Oversees community health outreach programs and maintains an inventory of all existing community programs and services. 5. Collaborates with other UMMS entities to cascade system priorities and objectives at a local level. 6. Establishes and maintains working relationships with major community organizations, public health departments, community health agencies, and area public educational systems. 7. Measures the effectiveness of community benefits programs designed to improve the health status of target communities served. 8. Manages the accounting and reporting of community benefit activities collaborating with departments and functions that are required to complete state and federal reporting of community benefit activities. 9. Assures compliance with state and federal regulations related to community benefits programming. 10. Prepares an integrated annual community benefit report and communicates activities to key internal audiences and external constituencies. 11. Identifies and pursues grant opportunities to sustain health promotion initiatives. 12. Develops and coordinates content for public web site dedicated to community outreach. 13. Patient Resource Center and Concierge Services (only for UMMC Campus) a. Manages the UMMC Patient Resource Center and Concierge Services b. Monitors and develops new service requirements to enhance patient and family needs 14. Population Health: Develops and enhances lifestyle management programs to support organizational goals related to population health strategies What You Need to Be Successful: Education and Experience 1. Master's degree in nursing, public health, or health care administration required. 2. Seven to ten (7-10) years' experience in community health work within a large health system. Knowledge and experience in conducting community-based health planning and programming with community health needs assessment experience. 3.
