Responsibilities

The Podiatrist will provide general podiatry services to the residents of nursing homes, assisted living facilities, and long term care facilities within an assigned territory.

Essential Functions:

  • Conducts comprehensive evaluations and regular podiatry care.

  • Provides wound care management.

  • Provides emergency care.

  • Enters patient information and completes required clinical documentation in the electronic medical record.

  • Spore testing on a monthly basis.

  • Performs other duties as assigned.

Qualifications

Education & Qualifications:

  • D.P. M.

  • Completion of a one year podiatric residency program.

  • Current State professional license.

  • Valid driver's license.

Skills:

  • Podiatry background with proficiency in the assessment, treatment, management and evaluation of podiatric care.

  • Ability to work independently on a daily basis.

  • Excellent written, verbal, interpersonal and organizational skills.

  • Ability to use email, the internet and to learn other healthcare related software.

You will collaborate with the clinical manager, case managers, and physician(s) to complete the patient care planning at the onset of care. You will have the opportunity to spend quality time with the patient, in their own home to assess the medical needs and recommendation for entry into services. * Performs patient start of care comprehensive assessment (using the OASIS tool), evaluation, development and implementation of Plan of Care. * Collaborates with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop a patient specific care and treatment plan. * Initiates appropriate referrals for other medical services when indicated. * Performs various types of nursing visits periodically to continue to monitor progress and evaluate services (Resumption of Care, Recertification, routine or follow up visits as needed.) * Participates in patient care conferences and leads discussion regarding new patients admitted into services.
