Act as primary and preventative care provider for patients in the practice.
Collaborate with physicians to provide patients with the best care possible.
Determine patients diagnosis, prescribe the medication to be utilized during the course of treatment, and provide education.
Review the appropriateness of the existing medication and consultation with the providers to prescribe and/or administer medication, monitors the effects of the medication on the patients mental and physical health.
Document prescriptions, interventions, weight, height, and blood pressure upon every visit.
Collaborative documentation on all required forms and documentation in the client record.
Performs other duties as assigned.
Bachelor's Degree in Nursing or equivalent
Current, valid New York State PA / NP License
Current CPR (BLS) Certification
Excellent oral and communication skills
Excellent time management and organizational skills
Excellent interpersonal skills required
Careful attention to detail / multitask
Bilingual (Chinese) a plus
