Position Summary

SUMMARY AND PURPOSE OF POSITION:The Practice Coach collaborate closely with the clinics to facilitate strategic implementations using change management skills, team development and Lean methodologies while improving quality health outcomes and increasing the efficiency of the practice. Their primary function is to navigate staff towards successful completion of all medical home initiatives and to coach managers, providers and staff, to develop the capacity for sustained change and improvement. Leads process improvement activities related to technology, workflow and application management in the ambulatory setting.A Physician Practice Coach will serve the role of:Quality Improvement ExpertCommunicatorCollaboratorSystem ThinkerFacilitatorEducatorChange LeaderCoordinator

Minimum Qualifications

MINIMUM QUALIFICATIONS Education, Certification, Registration, LicenseBachelor's DegreeField of Study: healthcare AdministrationField of Study: Related FieldYears of Experience: 3-5 yrs practice mgmt experience in lieu of degreeSPECIALIZED KNOWLEDGE:Bachelor's degree in a health care administration or related field or at least 3-5years practice management experience in lieu of degree

Thorough understanding of outpatient Quality Improvement principles andmethodologies

Excellent computer, communication and interpersonal skills

Practice operations experience

Broad understanding of all provider/clinical workflows, relevant applications and the role of technology within the ambulatory care setting

Ambulatory Care work experience

Patient Centered Medical Home Certification knowledge/experienceKnowledge of Epic or EMR system
