We are looking for someone to join our team that is revolutionizing the delivery of healthcare to vulnerable patients.
As a Physician, you will work as a provider of a number of high-complexity patients.
You will invest your time in personal patient home-visits that typically last about 45 minutes to an hour. This will provide you with more than enough time to make personal investments with your patients as you manage their medications, educate patients and their families on their disease(s), treatment plan, and future care plan.
You’ll also collaborate with your extended team of clinicians to make sure you are providing the most excellent care possible.

CFP&SM consists of three thriving primary care clinics with locations in Cary, Raleigh and Holly Springs, offering family medicine and sports medicine services. The primary position is an experienced CMA/RMA with a track record of: * Receiving and processing prescription requests via phone, fax or Surescripts * Assisting with scheduling of office visits and lab work * Completing prior authorizations for medications * Contacting patients by phone to provide requested follow up, advise them of test results or relay provider's instructions * Assisting in communication between patients and providers by phone or through Duke MyChart Knowledge, Skill Set and Abilities * Working knowledge of triage protocols * Expertise in refill protocols * Knowledge of current medications * Customer service and phone skills Overall Clinical Expectations * Maintain clinic-wide competencies to provide direct patient care (ex. ear irrigation, vaccine and medication administration, wound care and IV discontinuation (as delegated by the physician). * Ability to review/identify a patient's medical history and interpret basic medical data such as vital signs, height & weight, vision and chief complaint. * To collect, prepare and analyze patient lab specimens as directed by provider. * To instruct the patient (and family) on how to preform physician recommended preventative care, take prescribed medication, prepare for tests, monitor treatment progress and note symptom changes. * Contribute to CFP&SM and DukeHealth's goal of total quality improvement.
