POSITION SUMMARY Provides assessment, treatment and education of patients. Coordinates treatment with agencies and organizations for the delivery of care. Provides care in a professional, efficient and ethical manner, as well as in accordance with the standards and recommendations of Joint Commission, applicable laws and regulations of the State of New Hampshire, and philosophy of Elliot Health System (EHS). KEY RESPONSIBILITIES Patient Care#- Whether in the inpatient or outpatient setting, provides assessment of individuals by obtaining a comprehensive history, examines the patient to obtain objective physical data and supporting information through laboratory and imaging modalities, formulates a differential diagnosis and develops a reasonable treatment plan to include pharmacologic and non-pharmacologic interventions, making appropriate referrals as indicated. Collaborates with nursing, support personnel and administration in matters affecting patient care. Ensures the protection of patient rights and adherence to HIPAA regulations. Ensures appropriate handoff communications are accomplished. Documentation of Services#- Maintains ongoing documentation of patient interventions within the timeframe established by EPN/EPS policy, to include documentation of initial assessment, treatment plans, progress notes and any other patient interactions, to include telephone calls. Quality Improvement

  • Participates in Network QI activities that, at a minimum, are necessary for Joint Commission accreditation and to comply with the bylaws of the Medical Staff of the Elliot Hospital. Office/Unit Operations#- Works with staff to maintain the efficient operation of the practice, collaborating with nursing, support personnel, and administration in matters affecting the efficient operation of the practice. Participates in staff meetings and cooperates with practitioners and co-workers to achieve departmental objectives. Regularly attends departmental and State of the Network meetings. Regularly reads communications regarding network updates and projects and contributes to efforts as indicated. Patient Experience#- Communicates effectively with patients and their families regarding diagnoses, goals, treatment plans and medications to achieve patients# expectations, provide timely results and engages them in their care. Has not been subject to a formal review of a patient complaint by a hospital committee. CredentialingSubmits -#Credentialing and re-credentialing information to the Office Manager in a timely fashion once requested by Medical Staff Service Department. Policies and Procedures

  • Adheres to all Elliot policies and procedures including, but not limited to; infection control procedures, safety procedures, Universal Precautions, patient confidentiality and security policies. On-Call Responsibilities#- Participates in call coverage as required with other providers, which includes coverage of other practices as defined by the call group, Elliot Hospital, and nursing facilities. Responds to ED pages within 15 minutes. REQUIRED KNOWLEDGE # SKILLS Proficiency with EMR and excellent documentation skills. Excellent patient safety and quality skills Ability to deliver patient care in a manner that is appropriate to patient#s age, physical ability and intellectual capacity and that is culturally sensitive. EDUCATION/EXPERIENCE/LICENSURE Education: Medical degree from an accredited medical school. Residency and fellowship training as required for specialty from an accredited program. Experience: Certification/Licensure: Licensed to practice medicine in the State of New Hampshire. Board Certified/ Eligible or Admissible by the applicable governing Board for Physician#s specialty. The PA's primary responsibilities will be to support the Plastic Surgery division in the OR and ambulatory settings. The ideal candidate will provide patient care pre and post operatively, assist with procedures in the ambulatory setting, and First Assist in the OR. The PA's responsibilities will include closing incisions, performing physical exams, interpreting diagnostic tests, and counseling patients and families about treatment. The PA will assist the staff with handling urgent patient calls during the workday and provide backup for overnight and weekend call coverage. The ideal candidate should be experienced surgically and have excellent time management skills, a strong work ethic and a positive attitude.
