Overview

Our Physician is one who provides a full range of medical services for inmates. Provides required documentation of services to the Site Medical Director or designee in order to monitor provision of clinical services.

Notifies the Medical Director and H.S.A. regarding changes in schedule coverage. Assists in arrangements for coverage of medical services if unavailable for an extended period of time.

Qualifications

Education for Physician:

  • Medical school graduate

Experience for Physician:

  • Experience in Family Practice, Emergency Medicine, Internal Medicine or Public Health preferred (Board Certification)

Licenses/Certifications for Physician:

  • Current licensure as a Physician within the state

  • Current CPR Certification

  • Current DEA number

Responsibilities Reports to assigned facility at designated hour to examine referred patients. Conducts on-site sick call and chronic care clinics, as established by Medical Director.

Review and countersigns therapeutic orders and corresponding progress notes of inmates seen by the P.A./ARNP, if applicable, ensuring they are appropriate and effective. Visits the infirmary daily when on-site and records encounters in the patient's progress notes, as applicable. Ensures progress note documentation is in SOAP format, problem oriented, corresponds to the therapeutic order and is dated, timed and legible.

Responds to a code or health emergency within standard guidelines. Ensures all verbal or telephone orders are countersigned within one business day, if possible. Adheres to the established formulary for therapeutic regimens before utilizing non-formulary procedures.

Makes pertinent observations and draws logical conclusions, which validate need for non-formulary drug, which is recorded on non-formulary form and submitted to the Medical Director for approval. Utilizes available in-house resource personnel for treatment or resolution of identified problems before utilizing off-site referral, if possible. Provides emergency treatment on-site and responds appropriately in urgent or emergency situations.

Demonstrates proper technique for cardiopulmonary resuscitation and related drug therapy. Supports standards of medical care through adherence to existing policies and procedures for: admission to the infirmary (when available), transfer to emergency room and utilization review process for specialty consultant referrals. Supervises care given by other professional or non-professional personnel providing instructions as needed.

Reports any doubts or questions regarding the lack of appropriate referrals, nursing or medical intervention necessary for the realization of established patient goals to the Medical Director for disposition. Utilizes Universal or Standard Precautions at all times. Prescribes a planned regimen of total patient care, which is adequate, problem-oriented and appropriate to the needs of the patient.

Keeps patient information confidential and respects patients' rights to privacy in accordance with accepted confidentiality practices for incarcerated individuals. Provides input into facility mandatory committees such as P&T and Medical Audit Committee as requested by DON and H.S.A. Must maintain privileges as a member of the CCS Medical Staff Organization.

Must be able to obtain and maintain security clearance. Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations. Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.

Must be alert at all times; pay close attention to details. Must be able to work under stress on a regular or continuous basis. Post orders, if applicable, per site contract. Perform other duties as assigned.

Position Summary: We have an exciting opportunity to join our team as a Physician Assistant Fellow. The NYU Physician Assistant Fellowship in Cardiothoracic Surgery is an integrated 18 month clinical rotational program with a mission to train highly competent Cardiothoracic physician assistants with the skills to advance the specialty of Cardiothoracic Surgery. Selected fellows will have expressed a desire to gain expertise in not only clinical medicine, but also leadership, teaching and research. We expect our graduates to define and achieve proficiency in the practice of Cardiothoracic Surgery that will allow them to be future leaders, regardless of the institution in which they ultimately choose to practice their field. The PA Fellow is a post graduate PA trainee, with no prior formal work experience in Cardiothoracic Surgery as a practicing PA. At the conclusion of this training program, the PA fellow will be expected to sit for and pass the NCCPA CAQ exam in Cardiovascular and Thoracic Surgery. Under close supervision of a PA Preceptor and/or attending physician, the PA fellow is involved in the provision, coordination, and assurance of comprehensive medical care using sound medical knowledge and pertinent standards of care; performance of therapeutic, corrective, and prescriptive measures on the designated inpatient or outpatient population; and assumption of delegatory responsibilities as assigned by supervising and privileged physician. PA Fellows will become effective team members (and team leaders) who will work efficiently with nurses, ancillary services and consulting physician groups as part of a multi-disciplinary team. As a trainee in the department, all clinical responsibilities are conducted under the direct supervision of a PA preceptor and/or supervising MD. Job Responsibilities: * 1. Attends or completes at least 4 hours of didactic training each week: Didactic experiences will include administrative seminars, journal review, presentations based on the defined curriculum, monthly PA staff meetings, morbidity and mortality conferences, and research seminars The educational design will include problem-based learning, evidence-based learning, and computer-based instruction including the SIM lab to bridge didactic and clinical learning. * 2. Prepares for weekly conference in a timely and thorough manner through the completion of assigned readings, podcasts, and journals. * 3. Maintains mandatory attendance at weekly conferences. * 4. Actively prepares for and participates in SimLab procedures/codes/scenarios. * 5. Delegates CME allowance for participation in national societal conferences. * 6. Promotes own professional growth and development in clinical role. * 7. Upholds current expertise in area of practice. Additional Position Specific Responsibilities: II. CLINICAL As a trainee in the department, all clinical responsibilities are conducted under the direct supervision of a PA preceptor and/or supervising MD The PA Fellow will participate in an 18 month clinical core rotation: 1. Performs complete health history, physical examination and psychosocial assessment of patients within the designated practice or clinical unit. 2. Documents and communicates relevant patient information comprehensively and in a timely manner. 3. Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or as delegated by supervising physician 4. Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care and length of stay. 5. Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/family preference and enhances quality, access, and costeffective outcomes. 6. Prescribes inpatient and discharge medications according to New York State law, including controlled substances with DEA authorization. 7. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. 8. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges. 9. Performs delegated clinical duties effectively and efficiently. 10. Practices within the scope of practice of the supervising physician. 11. Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. 12. Educates patient and family to facilitate and promote healthy adjustment to new or changed health patterns. 13. Considers needs and behaviors of specific patient age and cultural groups in all patient care. 14. Participate in on-call Cardiothoracic Transplant schedule. Fellow is required to be within 2 hours of NYU Langone Hospital while on-call. III. CARE MANAGEMENT & COORDINATION 1. Supports the mission, philosophy, standards, goals and objectives of the institution, department, and clinical service. 2. Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. 3. Works collaboratively with the interdisciplinary team including the attending physician to develop and implement the therapeutic plan of care for a group of patients and ensure communication with family members. 4. Works collaboratively with the attending physician to maintain timely and appropriate communication with family members, house staff, other disciplines, and ancillary services. 5. Provides consultation about the therapeutic regimen to the nursing staff responsible for the patients care in the hospital setting. 6. Facilitates continuity of care across the continuum of services. 7. Efficiently manages patients throughput during admission and discharge. 8. Demonstrates knowledge of the organizations Core Values and incorporates them into the performance of responsibilities. 9. Participates in designated activities related to professional physician assistant practice standards. 10. Functions as a respectful member of the health care team. Minimum Qualifications: To qualify you must have a Competence: Graduation from an ARC-PA accredited PA Program Evidence of excellent interpersonal skills, effective communication skills, creative problem solving and critical thinking skills. Ability to facilitate group processes and work cohesively and collaboratively as member of an interdisciplinary team. Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities. Membership in professional specialty organizations desirable.
