The Advanced Practice Provider works under the general supervision of the Practice Supervisor and medical supervision of the physician(s) to provide excellent medical care completing the duties and responsibilities of the position while complying with HIPAA and CVPH confidentiality policies. S(he) is responsible to provide health care services as licensed professionals working within their defined scope of practice. This position requires regular use of independent judgment.


Patient Care:

A. Provide quality, compassionate and empathetic medical care to all patients as delegated by a supervisingphysician within the education, training and experience of the PA as delineated by protocols, practice guidelineand/or practice directives while meeting the productivity demands of the practice. Services may be provided in theoffice setting, nursing home(s) or CVPH Medical Center. This includes but is not limited to:

  • Obtain patient medical histories

  • Perform medical exams

  • Order or perform diagnostic and therapeutic procedures

  • Evaluate, diagnose and treat all patients per appointment schedule

  • Monitor the effectiveness of therapeutic interventions and treatment plan

  • Provide counseling and health education to patients and families as needed

  • Prescribe medications as needed and dispense medication samples when available

  • Make appropriate referrals

  • Provide emergency care as needed

  • Document patient records in a manner to support level of service being billed

Document all services, procedures and diagnoses provided to patient on charge ticker in accordance with established procedures to ensure accurate billing to patient accounts

  • Routinely consult with physicians

B. Assist in providing continuing medical information to nursing and clerical staffC. Assist in maintaining quality and safe infection control standards


A. Protect medical practice assets and resources within the control of the positionB. In conjunction with the Director of Practice Operations and/or Office Manager, assist with evaluation of effectiveness and efficiency of practice operations as it relates to patient flow, provision of medical care,

risk management and quality assurance.C. Assist in development, revision and implementation of practice guidelines and directives, risk management and quality assurance policies and proceduresD. Comply with HIPAA and CVHN confidentiality policiesE. Other duties as assigned

QualificationsCompletion of physician assistant education program approved by the American Medical Association.Current license as a Registered Physician Assistant or Nurse Practitioner in New York State required.Current or eligible for NYS DEA license required.Understanding and commitment to CVPH core values of teamwork, confidentiality and quality care.Excellent attention to detail and organizational skills

Expertise in clearly communicating medical information to professional practitioners and general public.Knowledge of OSHA standards/regulations.Ability to react calmly and effectively at all times including emergency situations.Legible handwriting.Above average spelling and memory skills.Develop and maintain effective relationships with staff, patients and external customers.Maintain confidentiality and support the goals and objectives of CVPH.Utilize available business technology to perform assigned duties.Ability to make appropriate decisions with a minimum amount of supervisory direction.Desire to remain current on emerging trends in technologies, techniques, issues and approaches.Ability to obtain appropriate privileges at CVPH.Ability to obtain credentialing by third party payers to enable third party billing.Current CPR certification.Must have ability to work in team environment.Medical office experience helpful.Experience in use of personal computers required; proficient in use of Word.Knowledge of medical practice management and/or electronic medical records software a plus.

VIEW JOBS 12/7/2018 12:00:00 AM 2019-03-07T00:00 Overview The Hospitalist Physician Assistant or Nurse Practitioner shall assist and supplement the Hospitalist Physician staff by rounding to assess, diagnose, and treat patients as enabled by their training and experience under the supervision and support of the Hospitalist Medical Director. These responsibilities will include caring for Hospitalist inpatients, responding to calls from the nursing staff, assessing changes in condition, responding to changes in condition, ordering tests and images, interpreting tests and images, and ordering medications. The Hospitalist Physician Assistant or Nurse Practitioner will also prepare discharges of patients and will prepare appropriate prescriptions, prepare discharge summaries, arrange follow up appointments and educate patients and families. Special Criteria Details*WAIVER REQUIRED, 4 Ten hour shifts per weekResponsibilitiesEvaluates patients to formulate a clinical impression/diagnosis/treatment plan, based upon the following parameters: Medical history, Physical examination, Laboratory and x-ray data, Other Medical records.Collaborates with supervising physician as clinically appropriate.Provide appropriate information to patients, families, and staff as necessary for implementing the plan of care.Collaborates with physicians, nurses, registered dieticians, and others as needed and appropriate.Maintains and completes medical records in a timely fastion.Participate in requested committees for department/hospital.Determine appropriateness of discharge home, admission to acute care, or admission to observation according to the individual patient's circumstances.Determine needs for consultations from other subspecialties.Develop and document appropirate discharge plans. This will include preparing discharge summaries, medical prescriptions, follow up appointments, education of patients and patient families, and communication with patient's primary care physician and nursing staff.Seek help, advice, or bedside assistance from the Attending Physician as appropriate based upon the patient's clinical circumstances and the Physician Assistant or Nurse Practitioner's training and experience.Participate in Interdisciplinary Rounds on R7.Work with supervising physicians to assure that all verbal and written orders of the Physician Assistant or Nurse Practitioner are signed by the end of each work shift.Participate in departmental/hospital wide quality improvement projects.Demonstrate a commitment to continued growth through ongoing education.Demonstrate knowledge of the Scope of Practice for Physician Assistants or Nurse Practitioners in New York State: CVPH Medical Staff bylaws; Hospitalist Program policies and protocols; and Standards of Medical Practice.Regularly attend hospitalist program meetings.Other duties as assigned.QualificationsLicensed as a NYS Certified Physician Assistant or Nurse Practitioner.ACLS required within 6 months.DEA privileges required.Prescription privileges required.
