General Summary:

Under supervision and in collaboration with the physician staff, the Advanced Practice Provider (APP) performs physical examinations and treats common episodic and chronic healthcare problems with an emphasis on health maintenance and disease prevention. This position is designed for healthcare professionals, such as nurse practitioners and physician assistants, who possess advanced training and expertise in their respective fields. The primary responsibilities include providing patient care in the clinic setting, performing in-office procedures, and focusing on patient education. As an APP, you will play a critical role in delivering high-quality healthcare services and ensuring excellent patient outcomes.

Essential Functions and Responsibilities:

  • Obtain a basic health history and comprehensive gynecological (GYN) history.

  • Perform a general physical exam, breast exam, and gynecological procedures such as Pap smears, pelvic exams, breast exams, and contraceptive management

  • Provide care across the reproductive health lifespan, including basic infertility services and menopause management.

  • Diagnose and treat a variety of reproductive health issues including but not limited to vaginal infections, sexually transmitted infections (STIs), menstrual disorders, family planning, menopause-related issues, HRT, and endometrial biopsy.

  • Perform and assist in reproductive health procedures, such as IUD insertions/removals, and Nexplanon placements.

  • Educate patients on reproductive health topics, including contraception, sexual health, fertility.

  • Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive and coordinated patient care.

  • Recognize common breast and GYN deviations from normal and provide management or referral as appropriate.

  • Recognize common non-GYN deviations from normal and provide management or referral as appropriate.

  • Provide preventive care, including screening for breast and cervical cancers.

  • Screen for contraindications and special conditions to contraceptive methods.

  • Provide appropriate contraception to patients, including intrauterine contraceptives (IUCs) and implants.

  • Test and treat STI's and other infectious conditions.

  • Provide PrEP and nPEP to prevent HIV infection.

  • Provide care for patients of all gender identities, including gender-affirming care.

  • Construct an accurate assessment and treatment plan based on history, exam, and lab findings.

  • Provide patient with general reproductive health maintenance, education, and counseling.

  • Prescribe medications and treatments as appropriate, following established protocols and guidelines.

  • Collaborate with multidisciplinary teams to coordinate patient care and ensure optimal outcomes.

  • Maintain accurate and up-to-date patient records in compliance with legal and ethical standards.

  • Participate in quality improvement initiatives, continuing education, and professional development activities.

  • Ensure complete and accurate medical documentation in EHR application and bill and code patient visits appropriately.

  • Participate in health center efforts to achieve established goals for productivity, with the minimum goal of 3.0 encounters per hour.

Assessment:

o Elicits and documents a complete health history.

o Performs and documents a complete physical examination.

o Order appropriate diagnostic tests.

o Assesses patient's perception of their health status, personal strengths, support systems, and patient problems that include psycho-social, physical, environmental, economic, and cultural factors.

o Identifies patients and families at risk for specific healthcare problems.

Diagnosis:

o Formulates diagnoses based on health history, physical examination, and diagnostic tests.

Planning:

o Formulates and documents health care plans with patients and their families, including treatment, health maintenance education, and counseling.

o Establishes, reviews, and documents short and long-term goals in collaboration with each patient.

o Plans comprehensive patient care with other health care providers.

Implementation:

o Demonstrates ability to provide appropriate care based on needs of a specific individual, including that individual's age.

o Institutes treatment plans based on assessments and interpretations of diagnostic tests and physical findings.

o Counsels patients according to their individual motivation and compliance.

o Provides telephone advice and triage of acute and unscheduled patients.

o Documents all pertinent data and interventions in the patient's records.

o Provides refills for routine medications.

o Institutes appropriate action when reviewing abnormal lab data and diagnostic tests.

o Consults with other providers, as indicated.

Evaluation:

o Evaluates the patient's understanding of and compliance with health care plan by regular follow-up and reassessment.

o Re-evaluates short and long-term goals with patients and participating health professionals.

Education:

o Patient and Family

§ Identifies the learning needs of the patients and family members.

§ Assesses the patient's readiness to learn.

§ Provides individualized health education.

§ Develops teaching goals and objectives that meet the patient's and family health care needs.

o Staff

§ Functions as a clinical resource person.

Continuity/Coordination:

o Functions as the health care provider for a specified group of patients.

o Coordinates patient care activities and health care planning among multiple providers, patients, and their families.

o Communicates and consults with family, staff and unit personnel prior to and during the patient's hospitalization. May also participate in discharge planning when appropriate.

o Promotes ongoing communication with community agencies to facilitate continuity of patient care.

Advocacy:

o Promotes the participation of patients and families in planning their care.

o Assists patients in obtaining information necessary to make an informed decision and supports patients in decision-making.

o Informs patients of the Patient Bill of Rights.

o Provides referrals to community resources when appropriate.

Qualifications

  • Nurse Practitioner:

Education:

  • BSN required; MSN preferred

  • Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner program

License/Certifications:

  • Current Registered Nurse, as defined by the California Board of Registered Nursing and successful completion from a college or university-based Nurse Practitioner program

  • Active and unrestricted Nurse Practitioner license in state of CA

  • Current Nurse Practitioner Furnishing License and verification of experience to furnish (if applicable)

  • Must hold current national board certification by first day of work and maintain

  • Must hold an active and transferable DEA license to California as required by the clinical specialty

  • Must be eligible for payer enrollment with CMS Medicare and Medicaid

  • BLS Certification (American Heart Association) required

Experience:

  • 2 years of experience in advanced practice or special preferred

  • 2 years in specialty area highly desired

  • Prior experience in a medical office environment preferred

Qualifications

  • Physician Assistant:

Education:

  • Completion of graduation from an accredited Physician Assistant Program

  • Must be certified by the National Commission on Certification and licensed in the State of California

License/Certifications:

  • Active and unrestricted Physician Assistant license in state of CA

  • Must hold current national board certification by first day of work and maintain

  • Must hold an active and transferable DEA license to California as required by the clinical specialty

  • Must be eligible for payer enrollment with CMS Medicare and Medicaid

  • BLS and ACLS (American Heart Association) required

Experience:

  • 2-5 years of PA experience preferred

  • 2 years in specialty area highly desired

  • Prior experience in a medical office environment preferred
