The Advanced Practitioner will partner with the St. Luke's Physicians to provide high quality, cost effective medical care. This individual will also participate in quality, patient satisfaction and performance improvement initiatives.
JOB DUTIES AND RESPONSIBILITIES:
Initial evaluation, or history and physical examination of neurology patients.
Performs in depth, systematic assessment of all assigned patients as requested.
Makes decisions regarding data gathering and appropriate management and treatment for the initial evaluation of a problem or the follow-up of a previously diagnosed condition; including requests for laboratory, radiographic or electro-diagnostic studies.
Performs therapeutic and/or diagnostic procedures as directed by the supervising physician.
Documents patient contacts appropriately.
Provides counseling, instruction and teaching regarding neurological problems to patients, families, colleagues and staff
Refers patients and/or families to appropriate support/community services as necessary.
Participates in performance improvement activities, departmental meetings, educational projects and research projects as requested.
Attends appropriate continuing education to meet or exceed hospital and state standards.
Assists with survey accreditation process.
Works in conjunction with Physicians to care for medical patients as requested.
Performs history and physical examination of all patients seen. Provides primary care consultations on all patients as requested by other physicians.
Round at least daily on all assigned patients.
Provide cost effective, high quality care based on best practice evidenced based medicine.
Make best efforts to adhere to standardized orders and practice guidelines that have been approved by the medical staff.
Provide regular, timely, and appropriate communication with referring physicians, patients and their families.
Participates in education of patients and their families.
Actively develops care plans and discharge dispositions in conjunction with the Case Management team.
Complies with established chain of command for work related problems.
Participates in regular meetings of the practice group.
Remain current with Medical Records documentation and respond in a timely manner to requests for chart review and completion.
Submit timely and accurate billing information.
Attends appropriate continuing medical education so as to maintain New Jersey State License.
Demonstrates ability to develop effective working relationships within the hospital and St. Luke's Health Network.
PHYSICAL AND SENSORY REQUIREMENTS: Sitting up to 6 hours per day and 2 hours at a time. Standing for up to 8 hours per day, 8 hours at a time. Walking for up to 6 hours per day, 1 hour at a time. Consistently lift, carry and push objects up to 10 pounds. Occasionally lift, carry and push objects up to 75 pounds. Transport patients weighing up to 250 pounds via wheelchair or stretcher. Frequently stoop or bend. Frequently reach above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, and high and low frequencies. Must be able to see as it relates to general, near, far, color and peripheral vision.
Graduation from ANA Accredited Nurse Practitioner Program with a Master's of Science degree. Current New Jersey licensure as a Registered Nurse and Certification from the State Board of Nursing as a Nurse Practitioner.
TRAINING AND EXPERIENCE:
Previous experience in neurology preferred. Minimum one year experience as Adult Nurse Practitioner.
Varying hours to meet the needs of the patients, including weekends and holidays.
