Summary of Duties
Responsible for providing and facilitating the daily care of acute and complex ill for the adult or geriatric patient from admission to discharge, as well as, co-managing pulmonary patients with collaborating physician(s).
Essential Job Functions
Rounds, evaluates patients, and formulates a plan of care or follows established patient care protocols with attention given to physical, psychosocial, education, and discharge needs of the patient/family.
Performs admission and discharge assessment and documents findings on appropriate record.
Initiates individual plan of care and discharge planning on admission, modifies the plan of care according to changes in patient condition.
Explains plan of care and provides appropriate education to patient and/or responsible person according to their predetermined level of comprehension, and documents on appropriate record.
Executes plan of care with multi-disciplinary Team.
Provides patient care discharge instruction to the patient and/or responsible person consistent with their level of comprehension and documents on appropriate records.
Performs specialized nursing skills as approved by the Alabama Board of Nursing.
Operate highly technical electronic life support, including but not limited to, monitoring equipment, defibrillator, and ventilators.
Responds immediately to life threatening situations and follows appropriate algorithm to sustain life, according to established protocols.
Participates in weekly patient care conference.
Participate in multi-disciplinary rounds with collaborating physician(s).
Maintain continuous availability of direct communication in person, telephone, or telecommunication with the supervising physician(s) to facilitate patient care and as may be required by law.
Maintain and protect patient confidentiality of medical records and related information at all times.
Consult and meet with the supervising physicians(s) and other personnel as may be required for chart review, quality assurance, or as otherwise necessary to ensure appropriate patient care.
Timely completion and accurate preparation of patient medical records by written or dictated systems.
Cooperate with and maintain professional behavior towards supervision physician(s), hospital staff, medical director(s), and co-workers in all work related activities.
Performs other duties as assigned.
Academic preparation and degree as a Physician Assistant with current licensure in State of Alabama.
One (1) year experience as functioning mid-level in an acute care hospital setting.
