POSITION SUMMARY Provides assessment, treatment and education of patients. Coordinates treatment with agencies and organizations for the delivery of care. Provides care in a professional, efficient and ethical manner, as well as in accordance with the standards and recommendations of Joint Commission, applicable laws and regulations of the State of New Hampshire, and philosophy of Elliot Health System (EHS). KEY RESPONSIBILITIES Patient Care

  • Whether in the inpatient or outpatient setting, provides assessment of individuals by obtaining a comprehensive history, examines the patient to obtain objective physical data and supporting information through laboratory and imaging modalities, formulates a differential diagnosis and develops a reasonable treatment plan to include pharmacologic and non-pharmacologic interventions, making appropriate referrals as indicated. Collaborates with nursing, support personnel and administration in matters affecting patient care. Ensures the protection of patient rights and adherence to HIPAA regulations. Ensures appropriate handoff communications are accomplished. Documentation of Services

  • Maintains ongoing documentation of patient interventions within the time frame established by EPN/EPS policy, to include documentation of initial assessment, treatment plans, progress notes and any other patient interactions, to include telephone calls. Quality Improvement

  • Participates in Network QI activities that, at a minimum, are necessary for Joint Commission accreditation and to comply with the bylaws of the Medical Staff of the Elliot Hospital. Office/Unit Operations

  • Works with staff to maintain the efficient operation of the practice, collaborating with nursing, support personnel, and administration in matters affecting the efficient operation of the practice. Participates in staff meetings and cooperates with practitioners and co-workers to achieve departmental objectives. Regularly attends departmental and State of the Network meetings. Regularly reads communications regarding network updates and projects and contributes to efforts as indicated. Patient Experience

  • Communicates effectively with patients and their families regarding diagnoses, goals, treatment plans and medications to achieve patients# expectations, provide timely results and engages them in their care. Has not been subject to a formal review of a patient complaint by a hospital committee. Credentialing Submits

  • Credentialing and re-credentialing information to the Office Manager in a timely fashion once requested by Medical Staff Service Department. Policies and Procedures

  • Adheres to all Elliot policies and procedures including, but not limited to; infection control procedures, safety procedures, Universal Precautions, patient confidentiality and security policies. On-Call Responsibilities

  • Participates in call coverage as required with other providers, which includes coverage of other practices as defined by the call group, Elliot Hospital, and nursing facilities. Responds to ED pages within 15 minutes. REQUIRED KNOWLEDGE # SKILLS Proficiency with EMR and excellent documentation skills. The Medication Nursing Assistant is directly supervised by the ACT Team Leader in collaboration with the licensed nurse on staff. The Medication Nursing Assistant provides support to individuals to maximize their recovery, ensure participant-directed goal setting, assist the individual served to gain hope and a sense of empowerment, and helps the individuals served become respected and valued members of their family and community. Requirements ESSENTIAL DUTIES AND RESPONSIBILITIES: * Process physician's orders (within scope of practice), request refills, monitor and provide access for self-administration of medication (regular timed administration and prn), monitor and record vital signs * Assist nursing staff with management of participant records, and scheduling of participant medical and lab appointments (internal as well as external). * Coordination of internal physicians schedules (all sites) * Maintains current knowledge base regarding any medication ordered. Investigate and research unknown medications before providing access for self-administration. * Checks all medication against physician order sheet. Checks participant medication at several intervals, including but not limited to when medications are received from the pharmacy, during weekly packaging and daily. * Customer Service Skills: General friendliness, a solution-focus attitude, being responsive and flexible * Participates in open, direct, solution-focused, communication with participants, families and team members. * Represents WestBridge in a professional, courteous, healthy and positive manner, including: wearing appropriate business attire. * Ensure program compliance with local, state and federal regulations. These include but are not limited to confidentiality, medication storage, administration and disposal. * Maintains a safe and orderly medication storage system according to state regulations, nursing guidelines, and WestBridge policies. * Medication collaboration and communication with pharmacies and insurance companies, as directed by the licensed nurse * Provides medication education to participants, families and staff. * Time spent in the community providing side by side support to participants (i.e. medication outreaches, doctor appointments, wellness, activities of daily living etc.) * Supporting participants to medical, dental, and wellness related appointments. * Collaborates with nursing staff to monitor AIMS tests, drug testing, Lab work, and primary healthcare needs of participants. * Communicates with treatment team daily and attend weekly staff meetings. * Assists individuals in developing and implementing Personal Achievement Agenda (PAA). * Gain knowledge of and implement evidence-based practices for treatment of people with dual disorders according to scope of practice. * Observe participants, document and report any unusual conditions, emergency situations, or difficulties with medication to the Licensed Nurse. * Obtains random urine specimens for drug screening and administers Breathalyzers as needed. * Manage the biohazard waste account including preparation for pickups and education of staff on the proper disposal process. * Completes documentation (ie; Electronic Medical Record, electronic timecard, expense reports, online trainings, etc.) filing, and chart maintenance in a timely manner and in accordance with company policy. * Maintains compliance with local, state and federal regulations, including but not limited to HIPAA and 42 CFR part II. * Adheres to all company policies and procedures. * Develops and adheres to a supervision plan with direct supervisor. * Attends individual supervision. * Protects confidentiality of all participants. * This is an hourly non-exempt 40 hour per week position. Work hours will be identified according to organizational needs in collaboration with the ACT Team Leader and Human Resources. Coordination with the team is needed for Holiday coverage. (Flexibility is required, specifically as it pertains to Admissions and/or medical needs of the organization). * Performs Certified/Licensed Nursing Assistant duties as needed. * Perform other duties as assigned. The requirements listed below are representative of the knowledge, skill and/or ability required. * Valid NH Licensed Nursing Assistant, in good standing. * Completion of a Board approved medication administration program conducted by a Board approved Registered Nurse * Minimum of two years LNA experience * Ability to carry out instruction and accurately document medical information. * Ability to work and communicate with others. * Knowledge and experience in the treatment of mental illness and substance-abusing populations as appropriate. * Knowledge of state and federal rules and regulations governing confidentiality. * Demonstrates a high standard of professional conduct and ethical behavior that will enhance the quality of care and encourage positive interactions among staff. * Valid drivers' license and appropriate vehicle insurance coverage of: $100,000 bodily injury per person / $300,000 bodily injury per accident / $100,000 property damage. * Successful pre-employment drug screening with negative results required. * Satisfactory Criminal Background Screening results. * Satisfactory Motor Vehicle Records results. * Active Crisis Prevention Intervention (CPI) Certification required or a plan for timely completion. * Valid Healthcare Provider (CPR/First Aid/AED) certification. * Regular and reliable attendance is required. * Computer skills including but not limited to, Internet access, word processing, report writing and spreadsheet applications necessary to generate appropriate reports allocated to the position.
