Evaluates and treats patients with disabilities, injuries, and/or diseases using physical, chemical and mechanical means in accordance with the written and signed prescription and/or referral of a state licensed physician or authorized practitioner. Maintains complete medical records for each patient.

Job Responsibility

  • Administers direct patient care in accordance with the written and signed prescription of the referring physician or other State authorized practitioner.
  • Utilizes appropriate evaluation techniques with efficiency and thoroughness.
  • Designs a treatment program to achieve established goals.
  • Makes accurate observations of patient tolerance and/or status during treatment and demonstrates the ability to modify a treatment program as needed.
  • Responds appropriately to established department emergency procedures for patient safety and care, when necessary.
  • Submits required documentation in accordance with the policies and procedures of the department.
  • Participates in quality improvement chart reviews and activities.
  • Attends family meetings/conferences and interacts with members of the inter-disciplinary staff pertaining to the welfare and management of assigned patients.

Job Qualification

  • Master's Degree required, or equivalent combination of education and related experience.

  • Current License to practice as a Physical Therapist in New York State required, plus specialized certifications as needed.

  • 1-3 years of relevant experience, required.

VIEW JOBS 11/23/2022 12:00:00 AM 2023-02-21T00:00 <p>As a Home Health Physical Therapist, you will:</p><ul><li><p>Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function.</p></li><li><p>Test/screen the patient's physical strengths to assist the physician in evaluating the patient's level of function and records findings to develop or pursue treatment programs and establish measurable training objectives.</p></li><li><p>Develop/implement a conditioning/rehabilitation program consistent with physician's Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.</p></li><li><p>Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing and revising the therapy treatment program and Plan of Treatment.</p></li><li><p>Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plan that reflects current plan of care as related to therapy, supervise/evaluate staffs' performance. Monitor the appropriate completion of documentation by physical therapy assistants and home health aides/personal care workers as part of the supervisory/leadership responsibility.</p></li><li><p>Accurately, promptly and thoroughly document patients' care observations, interventions and evaluations. Assure that interim (verbal) orders received from the physician and physical therapist are promptly and accurately documented, submitted for physician signature and implemented</p></li><li><p>Report patient's progress to the patient's physician, Clinical Manager, staff, patient and family.
