Responsibilities:

Duties and Responsibilities:

  • Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load.

  • Responsible for the correct reporting of billing information to the Director of Physical Therapy in regard to all patients on personal case load.

  • Provide direct patient care as prescribed, including evaluation and treatment planning.

  • Assist and assess transfers, gait and balance as required by functional mobility of patient level of assistance from minimum to maximum.

  • Manual manipulation, including pushing and/or pulling of patient body parts and limbs.

  • Responsible for the reporting and interpretation of treatment results.

  • Participate in development and presentation of in-services for the facility as requested.

  • Supervise support personnel in immediate work areas as appropriate.

  • Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare and Therapy Management Corporation's policies and procedures.

  • Participate in meetings regarding patient care as appropriate.

  • Assist when appropriate with any community education and public relations activities of the facility.

  • Provide education to patient's family as well as staff on the specific treatment plan as well as therapy goals for the patient.

  • Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.

  • Other duties as assigned.

Qualifications:

Qualifications:

  • A minimum of a Bachelors degree in Physical Therapy.

  • Current certification with the American Physical Therapy Association.

  • Licensed by the State in the designated geographic area in his/her respective therapy profession.

  • Demonstrates the ability to perform the following tasks without posing a threat to the health or safety of him/herself or others and without risk to self, co-workers, caregivers, or patient: lift, turn, move position and transport the patient into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.

  • Sedentary tasks related to job approximately 10%

  • Physical tasks as listed approximately 90%

  • Demonstrates the ability to perform a minimal assist transfer. Minimal assist transfers are defined for purpose of this document as the ability to lift and control 50 pounds.

This includes, but is not limited to patient care charting, charge entry, completing peer chart and peer reviews, and documenting on daily schedule. * Coordinates care of patients with other departmental and hospital activities in a manner that fosters a positive professional image consistent with the departmental and hospital goals and mission statement. This includes, but is not limited to, discharge planning, participation in care conferences, interdisciplinary rounds, collaborative practice groups, the student program and community endeavors. * Assigns, supervises and reviews the care of patients and duties of the Physical Therapist Assistant and therapy aide. * Promotes and supports positive and professional customer service, while interacting with clients, guests and hospital personnel. Works collaboratively with all members of the interdisciplinary team, other departments and disciplines to assure patients and customer's needs are met in a timely manner. Encourages the active participation of the patient and/or family in goal setting and patient/family training/education. * Provides positive and constructive feedback to enhance departmental operations. * Utilizes available information management systems to support departmental and hospital goals. This includes utilization of hospital wide e-mail, information announcements, policies, education, charge entry, and accessing patient care related information.
