Position Purpose: Evaluates and treats patients using the principles and practices of physical therapy in accordance with the patient#s age, developmental level and diagnosis. Essential Functions: Evaluates patients, establishes goals and develops individualized treatment plans.# Provides direct patient treatment based on current physical therapy practice and revises treatment plan based on the patient#s response. Provides education to patient and family regarding patient abilities and need for assistance and continued therapeutic intervention. Consults with physicians on patient#s evaluations, progress, and complications with treatment. Maintains medical and departmental records according to department policies and procedures. Participates in patient care conferences, department meetings and department and hospital in-services. Communicates with other professional staff and patient#s family/support system to provide continuity of care.

General Job requirements:


Bachelor#s in physical therapy Illinois PT license CPR Certification Experience in an outpatient setting#preferred. The physical therapist provides evidence-based direct and indirect physical therapy services to all patients referred to the department, promoting excellence with the delivery of care. Position Highlights: * The physical therapist (PT) is an active part of the interdisciplinary team. * The PT meets the requirements of all internal and external regulatory bodies. Position Responsibilities: * Executes a thorough patient evaluation, obtaining and interpreting data necessary to support a physical therapy diagnosis * Provides an evidenced base plan of care for treatment, including functional goals and methods to achieve said goals. * Adapts treatment environment, tools, equipment, and activities according to the needs of the individual * Monitors the individual's response to treatment interventions and modifies treatment and plan as indicated. * Determines the need to terminate patient services when maximum benefit or goals have been achieved. * Interprets the information provided by the Physical Therapy Assistant (OTA) and integrates the information into the evaluation and treatment plan as appropriate. * Delegates responsibilities to the PTA within guidelines of RUMC and the Illinois Practice Act. * Maintains treatment area, equipment, and supply inventory as required. * Documents concisely and accurately. * Demonstrates effective teaching and communication skills, especially as related for patient and family education. * Demonstrates ability for timely and accurate patient billing. * Maintains departmental productivity standards. * Demonstrates ability to safely lift and/or move therapy equipment. * Demonstrates ability to maintain patient safety. * Adheres to policies and procedures. * Supervises PT and other students as directed. * Provides proof of completed documents related to health and education requirements. * Participates in all quality improvement projects. * Identifies and pursues own professional growth and development. * Monitors own performance and identifies supervisory needs. * Functions according to the APTA guidelines, the APTA code of ethics, the Illinois Physical Therapy Practice Act and standards of the profession. * Maintains Illinois license.
