The Rehabilitation Services Physical Therapist will communicate timely, clearly and in a professional manner with staff, patients, patient family members, physicians and other internal/external customers. The staff member will demonstrate skills in listening, conflict management, system problem solving, decision-making and process improvement.

The therapist will practice in compliance with professional standards and professional code of ethics. The Physical Therapist is responsible for assuring that the PNHS values as defined by the PNHS mission statement are achieved through daily departmental operations. This includes, but is not limited to, direct patient care, professional growth and development, outcome/competency monitoring, compliance with professional organizations, regulatory standards and regulatory agencies; and participating in process improvements to advance patient care outcomes and satisfaction.

Participates in basic department development expectations for three professional pursuits, which must be some combination of the following: community event, clinical profession, a presentation and/or program, project or committee participation. Therapists in the residency program will be given ~4 hours/week of protected time to work 1:1 with their approved mentor. Education/Experience:

Bachelors, Masters, or Doctorate degree in Physical Therapy. Special Knowledge/Skills/Certifications/Licenses: 1. Licensed with the Minnesota Board of Physical Therapy 2.

Membership in the American Physical Therapy Association (APTA), and participate in the Minnesota Chapter of APTA, or section. 3. Individuals are responsible for the travel arrangements required to meet the expectations of the position. 4. Exhibit leadership qualities and serve as a professional role model within the clinic demonstrating excellent communication skills, strong drive for results, motivation of others and a high degree of integrity and trust.

Additional Information

This position is a hosted residency program in conjunction with Evidence in Motion. The cost of tuition for the EIM residency will be covered in full by Park Nicollet.

The Residency Program is approximately 1 year in duration. All residency related educational materials are included in the tuition. Any costs associated with transportation and/or lodging related to the Weekend Intensive component of the EIM curriculum is the responsibility of the resident.

APTA credentialed residencies require 150 direct 1:1 mentoring hours. There will be opportunities to mentor not only with the primary mentor, but other OCS, FAAOMPT, CHT and subspecialties while in the residency. There is no weekend or holiday work requirement for this position. 3 Resident positions available; various locations. Onsite interviews to be held in April.

VIEW JOBS 3/5/2019 12:00:00 AM 2019-06-03T00:00 Overview The Senior Services Home Care Physical Therapist plans, organizes, coordinates and controls a caseload of home care patients in assisted living and memory care communities. We are looking for someone to join our growing team. Senior Services is a division of Fairview Home Care and Hospice that treats patients living in the Ebenezer assisted living, memory care and independent living communities. We began our Senior Services Rehab Program 3 years ago and now support 24 plus assisted living communities throughout the Twin Cities Metro area in Minnesota. Our Rehab team has grown to 25 therapists currently who support these buildings in teams. In these settings our staff get to know each other well and have strong team collaboration, which improves communication and collaboration and in turn brings higher satisfaction to our patients. Recent employee engagement surveys from Senior Services physical therapists scored at 100 % asking if their work gives them a sense of personal accomplishment and they also scored 29 points higher on teamwork than the rest of their counterparts. This position will work with a team of OT's, ST's, RN's, PTA's, and Social Workers developing a collaborative care plan for the patient receiving home care services under Medicare part A. This role will support senior sites in various Ebenezer Sites mainly working at Towerlight in St Louis Park and Mpls Loren on Park and Park Towers. Home base will be at our Minneapolis office off 26th Ave. But will act in a Float PT role to occasionally help with evals, vacation coverage and admissions at other ebenezer sites as well. * This position is 80 hours per pay period on the day shift, could be 72 or 64 hours if applicant desired * M-F Day shifts * Full Benefits * Free continuing education unit (CEU) credits- Ongoing education and support * Supportive work environment * Laptops for electronic medical record documentation * Excellent benefits * Mileage reimbursement Responsibilities/Job Description To be successful the Physical Therapist with Home Care & Hospice one must be able to: Organizing/Staffing/Directing: * Accountable for managing the clinical episode of care to achieve desired patient outcomes and measurable functional goals. * Communicates consistently and effectively with patient, caregiver, significant others, physicians and all team members to assure appropriate implementation and completion of the patient plan of care. * Manages patient expectations. * Advances the POC with the multidisciplinary team. Consults with the other team members as needed to make modifications in the plan. Decreases utilization of services as patient moves towards goal achievement. * Supervises paraprofessional staff in patient care. * Attends and participates in patient care rounds. * Communicates/coordinates with Clinical Resource, Utilization Review and/or MA Specialists. Control: * Maintains complete, accurate, and timely clinical records/ documentation on all patients according to agency policy. Completes documentation on the day of service. * Meets all compliance measures (ex: MD orders, insurance authorization). Timely submission of all documentation. * Regular communication with manager regarding concerns and problem solving. * Fiscal Accountability: Professional Intervention * Provides safe, effective skilled and advanced intervention, within scope of practice according to patient need. * Provides counseling and teaching of patient, family and caregiver regarding care needs and health promotion/prevention. * Evaluates and documents response to care/services delivered. * Demonstrates ability to adjust approaches to care to reflect developmental level and cultural differences of populations served.
