The Physical Therapist evaluates and treats residents, under orders from a physician, through the use of a variety of therapeutic procedures designed to restore loss of muscle strength, coordination and body alignment, to relieve pain and to improve movement and function. The Physical Therapist participates as a member of the interdisciplinary team providing support and information within the area of Physical Therapy.

Responsibilities

1.Review available resident information and evaluate the impact of this information on the assessment and treatment process.

2.Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures. Assessment of home/vocational and equipment needs.

3.Develop and implement an individualized treatment plan based on resident needs and containing achievable functional goals.

4.Comply with evaluation, treatment and documentation guidelines and consistently maintain timely and accurate department records.

5.Instruct Assistants in treatment procedures to be utilized and provide close supervision while such staff members are working with residents.

6.Enter a progress note in the resident record following each treatment session.

7.Revise treatment plans, as needed, throughout the treatment process. Physical Therapist

8.Document the course of treatment in a clear concise discharge summary utilizing a prescribed format.

9.Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during any procedures.



  1. Work with nursing staff, resident and families to maximize resident's use of functional abilities.

  2. Instruct residents and family/other caregiver about exercise programs and therapeutic procedures to be continued by the resident at home following treatment.

  3. Assist in the referral process when a resident requires additional services following discharge. Maintain current knowledge of community resources.

  4. Attend and participate in, or provide information for rehabilitation team conferences regarding resident progress, problems or needs.

  5. Attend and participate in other department/facility meetings, as required.

  6. Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical competency.

  7. Participate in the quality improvement process.

  8. Promote the programs and services of Heritage Healthcare through formal and informal interactions with the community.

  9. Act in compliance with Heritage Healthcare's regulatory and professional standards and guidelines.

  10. Provide input to the Rehab Manager on clinical performance of assistants, as requested and appropriate.

  11. Adhere to facility policies and procedures and participate in facility quality improvement and safety programs.

  12. Perform other duties as assigned and required to provide quality care to residents.

  13. Understand reimbursement by various payers and allocates therapy resources efficiently in accordance with resident goals and outcomes related to discharge destination.

  14. Demonstrate a knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third party reimbursement guidelines.

  15. Support the company's commitment to Compassionate Care; projects a professional image to foster confidence in the Rehab Program.

  16. Perform duties in an independent manner and recognizes situations where direct supervision is needed; willingly helping others.

Qualifications

1.Bachelor/Masters of Science degree in Physical Therapy.

2.Current license in the state of practice.

3.Prefer supervision, training and/or experience in a setting serving the same age/type of resident population served by this facility.

4.Certification, if applicable, and demonstrated competency in identified areas of specialization.

5.Excellent oral and written communication and interpersonal skills.

6.Demonstrated competency in applying the principles, methods, materials and equipment used in physical therapy.

7.Ability to assess resident needs and develop and implement a comprehensive plan of care.

8.Knowledge of accreditation standards and compliance requirements.

9.Ability to complete the following activities safely, and at times, for prolonged periods:

  • Standing: Maintaining an upright position on the feet, especially for prolonged periods of time

  • Pushing: Applying pressure and exerting force against an object for the purpose of moving it forward or away from the source of the force - push or pull force up to 35 lbs.

  • Lifting: Raising an object from a lower to a higher position, or moving an object laterally, from position to position

  • Required to lift up to 50 lbs of force independently. May be required to lift over 50 lbs with assistance of another person or a mechanical lift

  • Stooping: Bending the body forward and down from the waist, hips and knees, sometimes for a prolonged length of time.

  • Reaching: Extending hand(s) and arm(s) in any direction away from the body

  • Balancing: Maintaining body equilibrium to prevent falling when walking, standing or crouching on narrow, slippery or moving surfaces

Job Summary Responsible for an assigned group of patients, typically geographically related and accountable for the oversight and overall management of the physical therapy services in the home setting. The physical therapist provides direct patient care and supervision of LPTAs in accordance with an established plan of care to address the patient needs, individual goals and physician ordered services. Essential Functions * Responsible for the daily management of an assigned patient caseload, ensuring timely care and documentation to meet the patent needs/services as prescribed in the established plan of care and per MD orders * Collects data pertinent to the healthcare consumer's health or the situation from patient assessments in the home setting, telephonic contacts and/or related EMR information available. * Analyzes the data to determine the issues and available resources to seek effective resolution. Identifies patients at high risk for re-hospitalization, falls and/or high utilization of resources that require additional intervention and case conferencing to ensure appropriate care and frequency of services mitigates risk and ensure high quality outcomes * Prioritizes multi-issue, situations appropriate to the criticality of patient health and welfare, patient/staff safety and impact to effective, timely services * Evaluates the PT patient to determine the rehabilitation needs, potential for achievement, and develops a plan of care in conjunction with the patient's physician. * Establishes and implements plan of care, performs PT services certified by the patient's physician, and directs the care coordination of a multidisciplinary clinical team approach, supervision of LPTAs and/or HHAs involved in the care of the assigned patient caseload to promote health and safe outcomes. * Maintains communication with multidisciplinary team providing updates and changes in schedules, patient data or needs, and physician orders. * Incorporates short and long-term goals established in the plan of care into therapy, as evidenced by documentation in visit notes, monthly progress notes, and discharge summaries. * Supports effective communication and collaboration with the physician oversight, and branch supervision to ensure timely feedback regarding patients' condition and/or need for changes in the plan of care * Remains and performs ordered treatments, therapies, and/or procedures in accordance with State Physical Therapy Practice Act and Home Health Conditions of Participation, JCAHO, Medicare Guidelines, OASIS and agency policy and procedures. Physical Requirements Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 50 pounds in handling patients, medical equipment, and supplies. Ability to travel extensively during all seasons to assigned patients place of residence. Works in a variety of home environments and conditions. Must speak English in good, understandable terms.
