Job Description

Ministry Employer Solutions industrial rehabilitation staff specializes in injury prevention and treatment of work and non-work related injury. The focus of safely returning the employee to work starts at the moment we begin treatment. Join our team of physical therapists, athletic trainers and occupational therapists and travel to client locations and help workers recover from their injuries, and also teach them new techniques that can help workers stay safe on the job.

This role is being based out of our Oshkosh 1855 S. Koeller Street or Menasha 1186 Appleton Rd. This role is based out of one location but requires travel between the two clinics and job sites. There is also the opportunity to start part time and grow to full time if desired.

Job Summary:

Evaluates and conducts medically prescribed physical therapy treatment programs.

Responsibilities:

  • Performs initial and on-going assessments of patient's condition.

  • Establishes, revises and evaluates a plan of care which is appropriate to problems identified and involves the patient/family.

  • Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.

  • Oversees activities of physical therapy assistants, students and other support personnel.

  • Coordinates patient care with other members of the healthcare team to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.

  • Provides education and resources to patient and/or patient's family based on clinical implications.

  • Documents all assessments, plans, interventions, and evaluations including service charges.

Qualifications

Licenses/Certifications/Registration:

  • Required Credential(s):

  • Licensed Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the state of Wisconsin obtained prior to hire date or job transfer date.

  • Preferred Credential(s):

  • Certified BLS Provider credentialed from the American Heart Association (AHA).

Education:

  • Bachelor's Level Degree

  • Bachelor's Degree or Diploma or Master's Degree or Doctorate in a Physical Therapy Program required.

Work Experience:

  • No experience required.
