The Physical Therapist/Physical Therapy will provide physical rehabilitative services to patients under specific medical orders as part of a coordinated team. Plan initial and subsequent treatment programs on the basis of clinical findings.

Participate in clinical conferences to share and coordinate the plan of care for patients. Other duties as assigned.

Legacy provides services solely to the senior population in Skilled Nursing Facilities, CCRCs, free standing Assisted Living and Independent Living Communities. We provide services under contract arrangement at skilled nursing facilities and in our numerous Medicare certified out-patient clinics.

Therapeutic techniques and programs incorporate principles of rehabilitation and healthy aging, emphasizing home and community independence. Our services are both rehabilitative and preventative with a strong component of health and fitness education.

1.Bachelors/Masters/Doctorate of Science degree in area of discipline
2.Current license in state of practice
3.Excellent interpersonal, fiscal, and clinical skills
4.Computer proficient

POSITION SUMMARY -Rehab and Therapy Services Dept. The incumbent in this position is a staff therapist who participates in the evaluation and treatment of patients based upon current principles and practices of Physical Therapy. These functions are performed in accordance with all applicable laws and regulations and Children's Health philosophy, policies, procedures and standards. Job Specific Responsibilities * Provide direct Physical Therapy evaluations and re-evaluations, including assessment of prior and current level of function, setting individualized goals and making discharge recommendations in accordance with standards of practice; provide direct PT treatments according to standards of practice, including patient and family education, progression of treatment plan and implementation of appropriate therapeutic interventions; report on patient progress through appropriate written documentation including evaluation forms, progress notes, education forms; and collaborate with physicians, patients, family and other members of the health care team relative to patient treatment and progress as well as other affiliated agencies with regard to effective discharge planning. * Coordinate and prioritize own schedule in relation to demands of patient treatment and other departmental responsibilities; supervise rehabilitation aides; mentor and instruct PT interns and other students as indicated. * Demonstrate proper use and maintenance of department equipment and supplies; attend and participate in departmental meetings, in-services, rounds and peer reviews; participate in program development as assigned by Team Leader; and participate in continuing education programs or other opportunities for learning for continued professional competency. * Ability to meet physical and non-physical demands as outlined in the job description. * Assume other duties and responsibilities that are appropriate to the position and area. * The incumbent will cover multiple outpatient locations throughout the metroplex. * The position is PRN -flexibility to be available Monday thru Friday.
