FTE 1.0 Position Summary: Responsible for planning, implementing, and evaluating physical therapy services for patients in concert with the total care plan. Provide quality care to attain patients optimal functional level.

Work directly with patients on a one-on-one basis during every visit. With our one-on-one treatment model, you will never be double booked, allowing you to provide quality care while building lasting relationships with your patients. 1:1 care for sessions, without double booking. Managerial/Supervisory Responsibilities:

None Essential Job Functions: Assess the patients physical therapy needs: Obtain a medical history, particularly as it is related to the present condition, from the patient, family, physician, team members and/or previous records.

Evaluate the patient in the following areas: ROM, muscle status, balance skills, coordination, functional activities, gait analysis, postural alignment, endurance, learning abilities and mental status. Assess the facility environment to determine and make adaptations as needed for the ease and safety of care and the maximal independence of the patient.

Assess equipment needs in the facility and assist in obtaining same. Evaluate the patients condition, family and home situation to determine what assistance will be available from family members in caring for the patient; determine if additional services or resources are needed. Assess equipment needs and medications for safety and progressive recovery at LLOC.

Instruct caregivers to special needs of the patient, including but not exclusive to positioning, transfers, and mobility function. Develop an individualized therapy plan. Interpret hospital and clinical services as the patient and family are involved in the development of a care plan.

Consult with appropriate disciplines and request their involvement in care, using their information and ideas to develop the plan.

Consult with physician and obtain consent to proceed with treatment plan. Provide physical therapy services and treatment consistent with POC, requiring specialized skill and ordered by the physician.

Provide physical therapy procedures and techniques which may include: muscle re-education, progressive muscle strengthening exercises, ROM, gait training, functional training, application of physical modalities, posture, breathing exercises, coordination and balance exercises. Modify treatment plan as indicated by patient condition progress. Instruct caregivers and family for maximal function and safety in the home environment, including proper use of equipment, body mechanics and guarding techniques.

Instruct, supervise and counsel caregivers and family members in continuity of care management. Make referrals to other disciplines as appropriate. Provide written feedback to referral source as appropriate Maintain appropriate and timely documentation to patient progress.

Administer planned an informal learning experiences for the patient, delegating appropriate reinforcement to other team members. Evaluate the therapy plan. Performs other duties as required Qualifications:

Education/Training: Masters or bachelors degree or a graduate of an accredited physical therapy school as approved by the American Physical Therapy Association in collaboration with the Council on Medical Education or the American Medical Association. Experience: License/Certification:

Current license to practice physical therapy in Washington State. Skills: Strong written, verbal and interpersonal skills.
