Evaluates and treats patients with disabilities, injuries, and/or diseases using physical, chemical and mechanical means in accordance with the written and signed prescription and/or referral of a state licensed physician or authorized practitioner. Maintains complete medical records for each patient.

Job Responsibility

  • Administers direct patient care in accordance with the written and signed prescription of the referring physician or other State authorized practitioner.

  • Utilizes appropriate evaluation techniques with efficiency and thoroughness.

  • Designs a treatment program to achieve established goals.

  • Makes accurate observations of patient tolerance and/or status during treatment and demonstrates the ability to modify a treatment program as needed.

  • Responds appropriately to established department emergency procedures for patient safety and care, when necessary.

  • Submits required documentation in accordance with the policies and procedures of the department.

  • Participates in quality improvement chart reviews and activities.

  • Attends family meetings/conferences and interacts with members of the inter-disciplinary staff pertaining to the welfare and management of assigned patients.

  • Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.

Job Qualification

  • Master's Degree required, or equivalent combination of education and related experience.

  • Current License to practice as a Physical Therapist in New York State required, plus specialized certifications as needed.

  • 1-3 years of relevant experience, required.

VIEW JOBS 9/23/2022 12:00:00 AM 2022-12-22T00:00 <p>As a Home Health Physical Therapist Assistant, you will:</p><ul><li><p>Administer the physical therapy treatment plan as developed by the Physical Therapist.</p></li><li><p>Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.</p></li><li><p>Accurately documents daily, weekly, or monthly records of clients'. Submits clinical paperwork to the office on time each week.</p></li><li><p>Attend educational meetings and coordinates and contributes to department in-services.</p></li><li><p>Notify physical therapist or Executive Director of any equipment failures, lack of supplies, staff or client changes, and any other departmental</p></li><li><p>Schedule clients and performs varied office tasks.</p></li><li><p>Apply concepts of infection control and universal precautions in performing patient care activities.</p></li><li><p>Accept clinical assignments that are consistent with education and competence to care for patients.</p></li><li><p>Promote compliance with all fiscal intermediaries and/or other third-party payors, through education, coaching, and other assistance as necessary.</p></li></ul><p>Required Experience/Skills:</p><ul><li><p>Associate degree in Physical Therapy from an American Physical Therapy Association.</p></li><li><p>Current state license to practice Physical Therapy.</p></li><li><p>Current driver's license. Must have reliable transportation and insurance.</p></li><li><p>Proof of current CPR certification.</p></li><li><p>Minimum two years experience as a Physical Therapy Assistant,</p></li><li><p>Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.</p></li><li><p>Self-directed, enthusiastic, and accepts constructive feedback.</p></li><li><p>Demonstrates good verbal and written communication and organization skills.</p></li><li><p>Communicates and demonstrates a professional image/attitude for patients, families, clients, coworkers, and others.</p></li><li><p>Meets mandatory continuing education requirements of Kindred at Home and licensing board.
