Physical Therapist

Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will be responsible for assessing, planning, and implementing individualized treatment programs for patients with various physical conditions.

This role requires a strong understanding of orthopedic practices, discharge planning, and the ability to work in diverse settings including inpatient, outpatient, and home health care environments. Responsibilities Conduct comprehensive evaluations of patients to determine their physical therapy needs. Develop and implement personalized treatment plans based on patient assessments and goals.

Utilize various therapeutic techniques including orthopedics and trigger point therapy to facilitate recovery. Educate patients and their families about treatment plans, exercises, and self-care strategies. Collaborate with other healthcare professionals to ensure coordinated care for patients.

Monitor patient progress and adjust treatment plans as necessary. Document patient care activities accurately in compliance with medical terminology standards. Participate in discharge planning to ensure smooth transitions from inpatient to outpatient or home community care settings.

Skills Proficiency in orthopedic practices and trigger point therapy techniques. Strong knowledge of medical terminology relevant to physical therapy. Experience in discharge planning for various care settings including inpatient, outpatient, acute care, and home health environments.

Ability to work effectively within a multidisciplinary team to provide comprehensive patient care. Excellent communication skills for educating patients and collaborating with healthcare professionals. Strong organizational skills to manage multiple patient cases efficiently.
