Physical Therapist

Hours:

32 hours per week, Monday through Friday or as determined by supervisor.

Director of Day Habilitation Programs

Location:

This position will serve three of our Day Habilitations in Fitchburg, Gardner, and Greenfield.

Qualifications:

• Must be licensed by the Massachusetts Board of Registration in Allied Health Professions.

• Acceptable CORI, SORI, and driving record background checks.

• Must pass Finger Print Analysis.

• Clear Record for the MA. DPPC Abuse and Neglect Registry and CT. Abuse and Neglect Registry (if applicable).

• Valid driver's License and reliable transportation required.

Requirements:

• Ability to lift 30 pounds.

• CPR/FIRST AID/AED certified,

• Ability to pass Proactive Approach Behavioral Challenges (PABC) to support staff in the event of a physical restraint.

• Ability to work collectively with multiple inter-disciplinary teams.

• Basic knowledge of Microsoft Word and teams, Excel, Power Point, and Office 365.

• Complete orientation week training within 90 days of hire and recertification as needed. 

Responsibilities:

• Completes formal PT. therapy assessments and evaluations as required by Mass Health Day Habilitation Regulations.

• Attends all required meetings (i.e. supervisory, DHSP, Interdisciplinary Team, Individual Reviews, and trainings.

• Provides staff training as needed (individual specific as well as orientation/annual trainings).

• Makes recommendations to Interdisciplinary Team for the development of therapeutic and functional goals and develops support strategies in DHSP.
