Company Overview The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

Essential Functions Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care. Provides services within the scope of practice as defined by the state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the health care team.

Observes and reports the patient's response to treatment and any changes in his/her condition to the supervising nurse and/or the physician. Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice. Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.

Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHC policy. Completes all patient evaluations and develops the PT plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care.

Education & Experience Licensure Requirements Current Physical Therapy licensure in state of practice. Current CPR certification required. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.

POSITION SUMMARY: <br><br> The Physical Therapist at OSF Home Care demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Coordinates inter-disciplinary care to patients, which may include nursing, occupational therapy, speech therapy, social work, home care aide, home medical equipment and home infusion services as needed. Formulates progressive care plans to restore function and prevent disability following illness or injury. <br><br> Provide patient-centered care, using critical thinking skills to ensure positive patient outcomes. The PT is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management, episode management and supervising of physical therapist assistants. Allows flexibility in their schedule to accommodate patient needs This positions will work 4 out of 5 Saturday or Sunday. <br><br> Qualifications REQUIRED QUALIFICATIONS: � Bachelor�s, Master�s, or Doctorate degree in Physical Therapy. � Requires current state license to practice as a Physical Therapist. � Current American Heart Association HealthCare Provider BLS is required prior to start date of employment. � Proficient with electronic documentation. � Provides own transportation with auto insurance and a valid driver�s license. Abides by all laws, rules, and recommendations to comply with OSF Safe Driving Policies while on duty. � Must be able to work independently in virtual work environment and execute own workload. PREFERRED QUALIFICATIONS: � One year recent physical therapy experience. � Home Health experience. � Experience with Microsoft Outlook, Word, Excel. � Knowledge of the PT State Practice Act and Home Care rules and regulations.
