The incumbent in this position is responsible for providing the full range of care and services within the scope of practice of the profession.
POSITION DUTIES: Performs Physical Therapy Evaluations and Re-Evaluations:
Initial evaluations and re-evaluations are completed within the appropriate time frame and contain all information required by departmental policy.
Performs Physical Therapy Treatments: Provides evidenced based treatments that utilize a variety of treatment techniques that are consistent with established goals, expected outcomes and age specific considerations . Efficiently manages caseload and meets established productivity target.
Effectively works with patients, family members, and other team members to provide for the needs of all patients.
Recommends necessary follow-up care and completes arrangements for equipment as needed.
Provides educational materials and training appropriate to the patient and family's level of understanding and motivation.
Documents Physical Therapy Interventions: Progress notes are concise and accurately reflect the interventions provided, the patients response to treatment and recommendations for continued care.
Daily charge are entered accurately and are consistent with documentation.
Participates Care Planning: Attends team staffings, rounds and other patient-related meetings.
Demonstrates collaboration in communication with team members and is open to feedback and receptive to change.
Supervision: May provide functional supervision to PTAs, Therapy Techs, and volunteers.
Serves as a Clinical Instructor: Provides clinical education to affiliating PT or PTA students as scheduled by the Coordinator of Clinical Education.
JOB SPECIFICATIONS: Education: Required: B.S.
in Physical Therapy Preferred: Masters Degree or DPT Experience:
New graduate physical therapist Preferred: Prefer one year experience in setting applied for.
Louisiana license in PT or eligibility for licensure, BLS: Healthcare Provider
Basic computer knowledge, proficient in English verbal and written skills.
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
