  Variable - shifts during week and weekends Location: 2300 Lone Star Road; Mansfield, TX 76063 The community youll be caring for: Join a family of caregivers focused on whole-person health and committed to making communities healthier. Provide wholistic care for patients that extends to the spiritual level by praying with patients and families.Chaplains are available to meet with patients, families and team members 24 hours a day, 7 days a week to provide spiritual support.

  The role youll contribute: Evaluates, develops an appropriate plan of care, and treats patients who present with bodily impairments and functional limitations as medically prescribed. The value youll bring to the team:
  
  Assesses patient ability to safely and effectively perform functional mobility skills and evaluate muscle, nerve, joint, and integumentary systems Designs and implements a treatment plan of care utilizing practice standards and recommended techniques and modalities, insurance guidelines, and patients needs. Monitors patients response and progress to treatment and modifies treatment plan as appropriate Documents and charges accurately, completely, and timely using measurable and objective terminology, following standards of practice, insurance guidelines and department/hospital policies and procedures Provides discharge planning assistance and information regarding appropriate selection/use of adaptive equipment and follow up services Oversees the care provided by physical therapy assistants and rehabilitation therapy aides, according to established regulations and practice standards Educates patients, family members, and caregivers about identifying needs The expertise and experiences youll need to succeed: Minimum qualifications : Graduate from accredited physical therapy program at the Bachelors, Masters, or Doctorate level Must hold and keep current annually Texas Licensure as a Physical Therapist.
