Physical Therapist - Home Health

Join our client's Home Health team as a Physical Therapist and make a meaningful impact on patients' lives. In this role, you'll deliver exceptional Physical Therapy services directly to patients in the comfort of their homes. Key Responsibilities:

• Conduct Physical Therapy assessments and develop personalized treatment programs
• Document and report patients' responses to treatment and changes in condition
• Educate patients and caregivers on therapy programs and device usage
• Supervise physical therapy assistants and, when needed, certified nursing assistants
• Prepare comprehensive reports and actively participate in staff and agency development activities

Schedule and Shift Details:

• Monday to Friday; 8:00 am/9:00 am to 5:00 pm
• One weekend per month (Saturday & Sunday; day shift with scheduled patients)
• Two holidays (one summer, one winter); no on-call duties

Travel:

• Local travel to residential homes

This is a pay per visit position

Requirements

• Master's degree or higher in Physical Therapy
• Minimum 1 year of experience in a healthcare setting
• Valid Physical Therapist license in the state of Illinois (IL)
• Valid regular driver's license
