Physical Therapist- Home Health 

We are seeking a Physical Therapist for a full-time and 100% Home Health focused opportunity in Portland, Oregon! The Physical Therapist will be focused on the West side of Portland, covering Beaverton, Hillsborough, Lake Oswego, West Linn, Tigard, and Southwest Portland. 

Compensation Package:

• Competitive Pay-Per-Visit Model
• 2.5 pts for SOC: $155-175
• 2.0pts for ROC, SCIC: $143 – 154
• 1.5pts for Evals, Recert and Agency Discharges: $112 – 130
• 1.0pts for Nonagency Discharges, Regular follow-up visits: $75-90
• 1.25pts for Reassessments: $75-90 Case
• Productivity Bonus Based on Weekly Metrics

Requirements:

• Physical Therapy Licensed in Oregon
• Prior Home Health Experience Preferred- Outpatient Experience, SNFs, Hospital Also Considered 
• 25 Points Per Week = Full Time- SET YOUR SCHEDULE

• Demonstrated ability to supervise and direct professional administrative personnel.
• Ability to market and deal tactfully with customers and the community.
• Has excellent observation, verbal, and written communication skills.

Responsibilities:

• Monday through Friday 8 AM to 5 PM
• Experience and Comfort with the Geriatric Population 
• Working with Medicare, Ortho, Neuro, Balance, and Vestibular Patients
• Order Equipment 
• Report to the Director of Rehab 
• Using Home Care Home Base HCHB - Training Offered 
• 4 to 6 Week Onboarding/Orientation for Successful Transition
