Physical Therapist - Home Health

Qualifications:

Required:

• Bachelor's degree or higher
• Over 1 year of Home Health experience
• ALL of the following valid licenses/certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • Physical Therapy in New Jersey (NJ)
• Full-Time position

Preferred:

• Over 2 years of experience in ANY of the following:
  • Community-Based Care
  • Clinical Setting

Overview:

Our client is seeking a full-time Physical Therapist in Ocean County, NJ, to provide home care therapy to individuals, helping restore function, improve mobility, and relieve pain.

The ideal candidate will have:

• A Bachelor’s Degree in Physical Therapy
• 1 year of experience in a homecare setting
• Ability to work well under pressure with strong attention to detail.

Key Responsibilities:

• Perform initial assessment or admission to the agency to determine patient treatment needs.
• Develop a plan of care with measurable goals and specific discipline-related treatments.
• Provide direct physical therapy services as outlined in the plan of care.
• Re-evaluate and revise plan of care as needed with physician approval.
• Facilitate total care services to patients by making appropriate referrals to other disciplines.
• Submit all documents promptly.
• Obtain physician approval to modify a current plan of care or treatment program.
• Perform other related duties as assigned.

Schedule and Shift Details:

• 8:30 a.m. - 4:30 p.m.
• 1 weekend per month
• Rotation of all national holidays.
