Physical Therapist - Home Health

- Home Health Job Number: 100200185 Employment Type: Fulltime Location: Tinley Park, IL 60477 Industry: Healthcare Salary: $55.00 Per hour Job Description: As a Physical Therapist within our client's Home Health team, you will play a crucial role in delivering top-quality care to patients within the comfort of their homes. Your responsibilities will encompass assessing patient functioning levels, developing tailored treatment programs, and providing ongoing support and education to patients and caregivers. Key Responsibilities:- Perform Physical Therapy assessments and design individualized treatment plans.- Document patient progress and communicate changes in condition effectively.- Educate patients and caregivers on therapy programs and equipment usage.- Supervise support staff as necessary, ensuring high-quality care delivery.- Participate in staff development activities and contribute to Agency growth. Work Schedule & Travel:- Monday-Friday; 8:00a/9:00a-5:00p  1 weekend per month (Saturday & Sunday; day shift w/scheduled patients); 2 holidays (1 summer / 1 winter); no on-call.- Local travel to residential homes. Requirements: - Valid Illinois PT license.- Master's degree in Physical Therapy.- Minimum 1 year of PT experience (no new grads).- Experience in a Healthcare Setting.- 1 year of Home Health experience preferred.- Familiarity with OASIS & HCHB preferred.- Regular Driver License (DL).
