As part of our client's Home Health team, the Physical Therapist will be tasked with delivering Physical Therapy services to patients within their homes, gauging patient functionality levels, and creating treatment programs to amplify independence and enhance physical/cognitive functioning. This role also encompasses patient and caregiver education, supervision, and monitoring of patient progress. Required Qualifications:

We are in search of a candidate who is armed with a Master's degree or above, holds a legitimate Physical Therapist certification/license in Pennsylvania (PA), possesses a regular Driver's License (DL), and can bring to the table at least a year of experience within a Healthcare Setting. Preferred Qualifications: Submission of your PT license number would be considered a plus.

Key Responsibilities: The chosen candidate will be expected to execute Physical Therapy assessments, devise treatment programs, document and convey adaptations to treatments and changes in the patient's condition, guide patients and caregivers through therapy programs and device utilization, oversee a physical therapy assistant, and if required, a Certified Nursing Assistant and create reports along with partaking in staff and agency enhancement initiatives. Schedule and Shift Details:

The ideal schedule runs from Monday-Friday, 8:00a/9:00a to 5:00p, with one weekend per month (Saturday & Sunday; day shift with scheduled patients) and 2 holidays (1 summer / 1 winter); there is no on-call expectation. Travel: The role includes local travel to residential homes.
