This role is essential for providing in-home physical therapy services, focusing on improving patients' independence and physical or cognitive functions. The ideal candidate will assess patients' levels of function, develop tailored treatment programs, and engage in education for both patients and caregivers. This position emphasizes the importance of collaboration and communication in enhancing patient care and outcomes. Key Responsibilities:

• Conduct Physical Therapy assessments and develop personalized treatment programs.

• Monitor and report on patients' responses to treatment and any changes in their condition.

• Educate patients and caregivers on therapy programs and the use of any necessary devices.

• Oversee the work of physical therapy assistants and, when appropriate, certified nursing assistants.

• Compile reports and engage in activities that contribute to the professional development of the staff and the agency. Qualifications:

• Must hold a valid Physical Therapist license in Illinois. Open to new graduates or those with less experience as a PT.

• A Master’s degree in Physical Therapy or related field is required.

• Must possess a regular Driver’s License for travel purposes. Schedule and Shift Details:

• Monday to Friday, 8:00/9:00 am to 5:00 pm.

• One weekend per month (Saturday & Sunday) with scheduled patients.

• Two holidays per year (one in summer and one in winter), no on-call duties. Travel Requirements: - Local travel to patients' residential homes. Benefits & Compensation: - Pay-per-visit position, with rates varying by service type and experience level. - $10K first-year success bonus.
