  Our client is seeking a driven and compassionate Physical Therapist specializing in Home Health to join their team. Qualifications: • Possession of a Bachelor's degree or above • In possession of valid certifications/licenses in American Heart Association Basic Life Support (AHA BLS) and Physical Therapy in New Jersey (NJ) • A minimum of one year of experience in a home health setting Furthermore, we prefer candidates who can showcase: • Over two years of experience in a community-based or clinical setting Role Overview: The chosen candidate will assume the responsibilities of a full-time Physical Therapist in Ocean County, NJ, with the primary role of providing home care therapy.
  
  Your role would involve rehabilitating individuals, enhancing their mobility, and alleviating discomforts. A successful candidate will possess an ability to manage work pressure, pay attention to detail, and showcase a Bachelor's Degree in Physical Therapy. Essential Duties: • Conducting an initial evaluation to understand patient treatment needs • Designing a plan of care that contains measurable objectives and specified treatments • Offering direct physical therapy services as stipulated in the plan of care • Periodically re-evaluating and revising care plans as necessary, with due approval from the relevant physician • Coordinating total care services for patients by making necessary referrals to other disciplines • Timely submission of all required documentation • With prior authorization from the relevant physician, making modifications to the plan of care or treatment program as required • Other duties as assigned Working Hours: • 8:30 am
