PURPOSE In accordance with a physician's order, assesses, evaluates, plans, and carries out physical therapy programs to restore maximum reasonable function and to minimize disability following disease, injury, or genetically acquired disabilities. To help the patient reach their maximal reasonably level of function and to cope with their limitation. JOB FUNCTIONS
1.Evaluates, assesses and sets up a comprehensive physical therapy treatment plan for each new patient according to their age. Assumes responsibilities for assigning appropriate other personnel to duties to carry out the treatment plan in order to reach short term and long term goals. To ensure proper documentation of all treatments carried out.
2.Supervises and reassesses patient treatment plans and documents progress in a timely fashion as determined by department policies. Reports progress to appropriate personnel and physicians.
3.Participates in discharge planning with related health professionals such as Social Services, M.D.'s, and other Rehab personnel. Provides recommendations based on observed evaluation and progress of patients to ensure safe and appropriate discharges.
4.Instructs patients in proper use of adaptive equipment such as wheelchairs and crutches and ensures that all persons involved understand instructions.
5.Provides and educates patients in appropriate exercise programs and coping skills. Adapts conventional Physical Therapy techniques to meet the age appropriate need of each patient along with other specific needs based on his/her mental, physical, and emotional status.
6.Provides education to related health professionals and/or the public in regard to physical therapy techniques, body mechanics, and assistive techniques.
7.Supervise and train physical therapy students as requested.
8.Appropriately assign and supervise specific treatment procedures to physical therapy assistants or aides.
9.Facilitate interpersonal relationships.
Education: Graduate of an accredited school of Physical Therapy. B.S., Certificate or M.S. in Physical Therapy required.
Licensure: Current licensure as a Physical Therapist in the state of Indiana. Must also have current CPR certification.
Experience: Satisfactorily completed three clinical affiliations.
Proficient in basic ortho/neuro/ped evaluation and treatment skills. Must be receptive to patient education process and creative in planning, teaching and setting goals for patients and students. Must possess good oral and written communication skills and proficiency in documentation.
