Physical Therapist - Part-Time/Prn - Part-Time 1St Shift

As a Physical Therapist, you will work with the interdisciplinary team to establish appropriate goals and creatively execute treatment. More importantly, you will help your patients achieve critical outcomes while acting as a trusted partner to the community you serve.

Position Summary

The Physical Therapist restores patient's function, alleviates pain, and improves safety, allowing the patient to continue to remain living in the comfort of home. In the home health care setting, the Physical Therapist functions as a piece of the interdisciplinary team, assisting in the patient's coordination of care.

In this role you will: Develop and implement an individualized treatment plan based on the patient's needs and achievable functional goals.

Instruct patients and family/caregivers about exercise programs and safety with mobility within the home. Communicate with patients, staff, family/responsible party, physicians, and related individuals regarding any changes, problems, concerns, and/or special needs the patients may have. Travel to, and treat the patient, within their home setting.

Qualified candidates will have: Graduate from an Accredited School of Physical Therapy approved by the State of Nebraska. Possess a valid PT license from the appropriate state.

This shall include maintaining required CEU's. Experience in Home Health Care preferred but not required. Must be willing and able to become CPR Certified.
