VIEW JOBS 12/1/2018 8:00:59 PM 2019-03-01T20:00 <p>Qualifications and Description of Duties <br /> <br /> <strong>A person is licensed as a physical therapist by the state of Massachusetts, and</strong><br />  </p> <ul> <li>Has graduated from a physical therapy curriculum approved by: <ul> <li>The American Physical Therapy Association; or</li> <li>The Committee on Allied Health Education and Accreditation of the American medical association; or</li> <li>The Council on Medical Education of the American Medical Association and the American physical therapy association</li> </ul> </li> </ul> <p><br /> <strong>If trained outside the USA :</strong><br /> Was graduated since 1928 from a physical therapy curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Confederation for PT<br /> Meets the requirements for membership in a member organization of the World Confederation for PT.<br /> <br /> <strong>RESPONSIBILITIES</strong>:<br />  </p> <ul> <li>Assists the physician in evaluating patient’s level of function, establishing treatment plan and goals.</li> <li>Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, as an appropriate coordinating plan of care with the physician as needed.</li> <li>Prepares clinical and progress notes and required summaries, as appropriate.</li> <li>Revises the plan of treatment and contacts the physician for change in the plan of care.</li> <li>Advises and consults with patient’s family and other healthcare staff.</li> <li>Coordinates care with agency DON and other staff as appropriate.</li> <li>Observes, records and reports any changes in the patient’s condition and updates the agency on changes in the patient’s status or schedule changes.</li> <li>Supervises the Physical Therapy Assistant including but not limited to, informing the PTA of new patients, scheduling supervisory visits and co-signing PTA documentation.</li> <li>Instructs patient and/or family in use of equipment safe patient management and home therapy programs as needed.</li> <li>Provides staff in-services when required.</li> <li>Participates in chart reviews, case conferences peer review and QA/PI activities when required.</li> <li>Communicates effectively with a patient, caregivers, and families as well as agency staff.</li> <li>Provides quality and appropriate care and reports issues or concerns to the DON /Administrator.</li> <li>Assures that all tasks are performed in a timely manner.</li> <li>Performs other duties as assigned.</li> </ul> Rehab At Home Webster MA
