Physical Therapist, PT,for a per diemopportunities performinghome healthvisits for ourvisits office, located inHilo, HI.This office services adult and geriatric clients on a per visit basis in territories throughout Hawaii county. One year prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist include:

  • Make home visits to clients in designated geographic territories.

  • Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.

  • Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.

  • Continually assess and revise the Physical Therapy

  • PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
  • Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.

  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualificationsfor a PT - Physical Therapist include:

  • A current Hawaii Physical Therapist
  • PT license.
  • A minimum of one year of recent work experience as a Physical Therapist
  • PT.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.

  • Ability to work independently and manage time effectively.

  • Strong interpersonal, organizational, and problem solving skills.

  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.

ESSENTIAL JOB FUNCTIONS As a member of a multidisciplinary team, provide support to families using a Primary Service Provider model and evidence-based coaching techniques. Daily Responsibilities: * Adheres to all Department of Health-Early Intervention Section (DOH-EIS) and ESH guidelines * Participates in the planning and implementation of a comprehensive and developmentally appropriate program for children with special needs and their families. * Participates as an evaluator and member of a multidisciplinary team that determines eligibility for Early Intervention (EI) services. Supports the child and family through the evaluation process. * Administers, interprets, and shares results from physical therapy assessments. * Provide Care Coordination services in accordance with DOH-EIS guidelines, if selected by the family. * Participates in the development of the Individualized Family Support Plan (IFSP) with the family and other team members. * As part of an IFSP team, helps families develop outcomes that are functional and based on childrens and families needs and family-identified priorities. * Pre-plans and debriefs with teammates in order to effectively coach families through routine-based activities which incorporate communication skills, motor skills (gross/fine), adaptive skills, cognitive skills, and social emotional/personal social skills, utilizing a Primary Service Provider model. * Provides consultative services to parents and other team members; interprets and applies physical therapy techniques; ensures proper follow through and competence with techniques using a coaching approach. * Provides ongoing monitoring of the child and familys needs, concerns, and strengths and assists with linking the child and family to appropriate resources as needed. * Establishes and maintains cooperative and collaborative relationships with families and community agencies, public and private. * Maintains billable hours as outlined by the DOH-EIS and keeps participant records in accordance with Federal, State, and ESH requirements. * Attends, facilitates, and participates in staff and team meetings. * Provides services within natural environments (e.g. home, day care and community). * Attends and participates in transition meetings. Other Responsibilities: * Serves as the supervisor for students and/or volunteers within the program. * Assists in maintaining a clean, hygienic, safe and childproof environment. Reports any safety concerns to the Program Manager. * Participates in training sessions and continuing education. * Participates in and/or coordinates ESH events (such as program activities, community fairs, fundraising, etc.), as requested. * Other duties as assigned. WORKING CONDITIONS Scheduled Hours: Monday through Friday, occasional weekend hours, early evenings and morning, as directed by Program Manager Equipment Used: * Program materials and equipment * Computers, mobile and respective software and applications * Multi-line Telephone with Intercom System * Copy and Fax Machine QUALIFICATIONS Minimum Education & Experience Equivalent To: * Bachelors degree in Physical Therapy from an accredited college or university program. * Current State of Hawaii license in Physical Therapy. * Current First Aid and CPR certification. * One (1) year minimum experience in providing physical therapy service is highly preferred. * Experience working with children with special needs in the field of early intervention is highly preferred. Skills & Abilities: * Significant knowledge with developmental disabilities, familiarity with ADA and Individuals with Disabilities Educational Act (IDEA) guidelines, and inclusion. * The ability, understanding and compatibility to interact with families, professionals, and paraprofessionals within a Primary Service Provider model of service delivery, using an approach that involves giving and receiving peer coaching. * Understanding of and respect for children with special needs and their families. Ability to be attentive to input from families regarding their childs preferences by utilizing a strength-based approach with culturally competent practices. * Positive public relations attitude and approach when interacting with those we serve and the general public. * Organizational ability to work independently and take initiative to complete assignments. * Clear and concise verbal and written communication skills and the ability to communicate effectively with people at all levels. * Operating knowledge of a computer, applications and software, standard office equipment and Microsoft (Outlook, Word and Excel). Must be able to use an iPhone or work cell phone if applicable. Mental Requirements: * Ability to work calmly with behavioral and health related incidents. * Alertness to respond to audio and visual cues from participants and their families, other staff members and the environment. * Ability to handle multiple tasks, adhere to timelines, and process urgent projects while maintaining an attention to detail. * Ability to respond appropriately and immediately to emergencies as defined by ESH Emergency Procedure Guide/Safety Manual. * Logic and reasoning for forecasting future needs, goals, and events. Physical Requirements: * Frequent bending, reaching, squatting, kneeling, twisting in order to observe, assess, and interact with clients. * Ability to sit in office chair and typing for periods of up to one hour without a break. * Ability to sit on the floor in families homes to administer evaluations that can last up to 90 minutes. * Occasional lifting, carrying, and loading/unloading toys and materials up to and including 25 lbs. to 50 lbs. used in home visits. * Visual and auditory ability to work with clients, staff and others in the workplace continuously. * Frequent speaking and listening (50%) to clients, staff, and other professionals in meetings and on the phone. * Frequent driving (to and from office and client homes). * Frequent exposure to disability disorders in clients; occasional emergency situation; occasional exposure to trauma; constant client contact and decision making; constant concentration required when working with clients. * Frequent work inside client homes; occasional work in outdoor settings. Conditions of Employment: * Ability to obtain and maintain criminal record clearance through the Department of Justice (DOJ). The HR Department must analyze DOJ/FBI live scan reports in accordance with applicable Federal, State, and Local laws, as well as fitness for the position. * Ability to travel to multiple work sites and client homes; must have access to a reliable personal vehicle. * Valid drivers license with an insurable driving record * Proof of current vehicle registration and safety check. * Must pass health screenings, obtain vaccinations and clear annual TB testing based on company policies. Freedom from any communicable disease or medical conditions that would interfere with the physical requirements of this position. * Ability to obtain and maintain clearance through the Office of Inspector General. * Ability to obtain and maintain First Aid and CPR certification. * Must attend all ESH required training.
