POSITION:

To assist in providing appropriate aspects of quality patient care under the supervision of licensed therapists.

QUALIFICATIONS/REQUIREMENTS:

  • Graduate of an approved Physical Therapy Assistant curriculum.

  • Is in good standing with all regulatory agencies and licensing boards.

  • Working knowledge of Medicare and other payor sources.

  • Full knowledge of resident's rights.

  • Current, active license as Physical Therapist, and is able to practice unencumbered.

  • CPR Certification, as required for license renewal.

  • Experience in long term care preferred

  • Able to work on multiple tasks at the same time.

ESSENTIAL JOB FUNCTIONS:

  • Implements and executes on established patient treatment plan under the supervision of a therapist.

  • Communicates with patients, families, referring physicians and other members of the team.

  • Provides timely written documentation per facility and department requirements.

  • Develops and enhances clinical and professional skills through knowledge and professional associations.

  • Communicates with patients, families and other clinical staff of treatment variations.

  • Provides timely written documentation as required by insurance provider to meet licensing, certification, regulatory, reimbursement requirements and physician treatment plan.

  • Supports and participates in department operations and development.

  • Reports any changes in patient status or needs to supervising therapist on a timely basis.

JOB FUNCTIONS:

  • Prepares and submits patient charges accurately to provide appropriate billing information.

  • Provides patient screens upon admission by identifying appropriate interventions for treatment.

  • Uses professional judgment to ensure safety of self, patients and others at all times.

  • Achieves quality patient care through interdisciplinary co-op, teamwork and effective communication.

  • Attends required meetings as designated by the Director of Rehab and/or Regional Manager.

  • Effectively communicates with Therapists concerning patients response to treatment plan. Provides constructive feedback to rehab techs/aides.

  • Attends in-services, training sessions or other educational presentations.

  • Coordinates treatment goals with other team members.

  • Maintains valid licensure, certification or equivalent as requested by appropriate state agency.

  • Maintains awareness of patient and community needs, conditions, and programs.

POSITION Provides and directs all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to organizations guidelines, professional standards and community needs. Serves as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers. Participates in quality assurance and customer service activities. ESSENTIAL JOB FUNCTIONS: * Makes evaluations that are initiated per M.D. orders-payer authorization and facility/payer source guidelines. * Develops treatment plan and provide treatment, consistent with benefit and reimbursement level for best functional outcome for patient. * Educates staff, other health care professionals and third party payer on the benefits and functional outcomes of intervention. * Appropriately and effectively completes documentation including evaluations, daily progress notes, summaries, or monthly reports as required. * Screens all individuals who may benefit from therapeutic intervention. * Uses professional judgment to ensure safety of self, patients and others at all times. * Achieves quality patient care through interdisciplinary co-op, co-treatment, setting up protocols and programs. * Attends continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques. Provides inservice training sessions or other educational presentations to company, team or facility in order to enhance one's clinical and professional knowledge and skills. * Provides assistants and rehab techs clear directives concerning patients and treatment plans prior to initiation of patient care. * Confers with Program Specialists and supervisor to develop and implement new programs/techniques to enhance program utilization. * Maintains appropriate state licensure, certification and credentialing. * Submits billing, payroll, and expense documentation in a timely manner. * Makes appropriate referrals to other members of the rehab team.
