Assists in the provision of screens, collection of assessment data, and direct treatment to a general rehab caseload under the direction and supervision of a Physical Therapist. Provides caregiver training. Involvement in rehab program development. Where allowed by law, supervises rehab technicians and/or rehab aides.


Associates Degree from an APTA accredited program and licensure in the state of hire, where required by law.


Two years clinical experience preferred.

  • Conducts resident screens (without interpretation) as directed by the supervising Physical Therapist.

  • Collects assessment data as directed by the supervising Physical Therapist.

  • Under a physician's order, provides direct treatment to patients as directed by, and according to the plan of care outlined by, the supervising Physical Therapist.

  • Provides clinical documentation on a timely basis.

  • Provides patient and caregiver training according to the plan of care outlined by the supervising Physical Therapist.

  • Prepares materials and assists with other clerical duties as directed by the Physical Therapist.

  • Participates in quality improvement programs.

  • Ensures the delivery of quality rehab services.

  • Demonstrates courtesy, compassion, and respect to patients, caregivers, customers, physicians, and co-workers.

  • Completes required educational training per Daybreak policy, Maintain Compliance with Daybreak's Compliance Program and plan, adhere to the Daybreak's Code of Conduct,

  • Work with the facility and organizations on case management needs and requirements.

VIEW JOBS 10/4/2019 12:00:00 AM 2020-01-02T00:00 Hill Country Transit District Job Description Job Title: Rural Bus Driver Department: Rural Operations Reports To: Director of Rural Operations FLSA Status: Non-Exempt Salary: $11.30/Hour Revision Date: January 1, 2015 Effective Date: April 22, 2014 SUMMARY Responsible for providing transportation services to all individuals, including persons with disabilities, in a safe and timely manner, and for properly operating and maintaining the transit vehicle assigned. Responsible for complying with all Hill Country Transit District (HCTD) policies and procedures and all funding source regulations and requirements. ESSENTIAL JOB DUTIES 1. Must be able to follow a daily demand response service route. 2. Complies with all routing and scheduling directives. 3. Performs pre-operation safety inspections; coordinates all major vehicle servicing and repairs verbally and in writing as necessary through the HCTD Fleet Services Manager. 4. Responsible for routine servicing of vehicle and fueling at local service stations. 5. Maintains cleanliness and appearance of vehicle, both inside and out. 6. Maintains all required passenger and vehicle records. 7. Operates transit vehicles in a safe and efficient manner. Practices safety at all times in performance of required duties. 8. Operates other bus-related equipment to include wheelchair lifts, ramps, phone, and computers. 9. Assists customers who require special assistance and persons with disabilities as required by the American with Disabilities Act (ADA), including the securement of mobility aids and physically evacuates passengers in any emergency situation. 10. Provides information to the public regarding transportation services and transit policy. 11. Provides information to Lead Driver and/or Director of Rural Operations, and dispatch regarding route problems or traffic changes. 12. Reports passenger problems to Lead Driver and/or Director of Rural Operations for resolution. 13. Collects appropriate passenger fares. 14. Other duties as assigned. KNOWLEDGE, SKILLS AND ABILITIES * Knowledge of the rules, regulations and standard operating procedures for motor coach vehicles. * Ability to safely operate HCTD transit coaches and vehicles. * Ability to resolve customer complaints courteously and tactfully. * Ability to work under stressful situations. * Ability to communicate effectively, and follow oral and written direction. * Ability to prepare accurate records and reports. * Ability to establish and maintain effective working relationships with staff, other agencies and the general public using tact, courtesy and good judgment. * Ability to perform all essential functions of a motor coach operator on an emergency basis. * Ability to successfully complete all operator training programs as required by HCTD, state and federal rules and regulations. * Ability to work independently with minimal supervision. * Physical ability to perform the essential functions of the job. Duties are performed while sitting for long periods of time in a motor vehicle. Duties also require periodic standing to assist passengers. Employees may be exposed to road hazards related to adverse weather conditions or vehicular traffic. The employee may be exposed to fumes or airborne particles. The noise level in the work environment is usually moderate. Duties may be performed outside of normal working hours at varying hours of the day, including weekends and holidays. PHYSICAL DEMAINDS While performing the duties of the job, the employee is regularly required to talk. Must have good hearing and sight, including good depth perception, color vision, close vision, distant vision, and the ability to adjust focus. Must be able to bend, stoop, twist, turn, walk, climb, crouch, stand, and kneel. Moderate bending and crouching may be required to assist passengers in boarding and exiting the transit vehicle. Must be physically capable of assisting passengers who use assistive walking devices, wheelchairs or other special equipment. Must be able to frequently lift and/or move up to 50 pounds. Must be willing and able to administer CPR and first aid. REQUIRED QUALIFICATIONS * High School Diploma or Equivalent * Must be 21 years of age. * For positions that require the ability to operate a vehicle with a capacity of 16 passengers or more, must have a valid commercial drivers' license (CDL) Class B with Passenger Endorsement, or the ability to obtain Passenger Endorsement within 30 days of hire date; or * Must have a valid CDL permit and have already successfully passed the following (5) skills tests and be ready to take road test and take Passenger Endorsement and Air Brake Endorsement within 30 days of hire: * General Knowledge Test * Passenger Endorsement Test * Air Brake Test * Pre-Trip Test * Special Requirement Test * Regular attendance and reliability is critical to business operations. * Available and willing to work varying shifts, hours and days as assigned to include early mornings, late evenings, weekends, and holidays. * Must be willing and able to drive special transit vehicle, minivans, or 15-passenger van. * Have no more than two (2) moving violations in the past 12 months. * Must have reliable means of communication, i.e., home telephone, cellular phone. * Public bus driving experience highly desirable.
