What You Will Do:

As a home health physical therapist assistant, you will provide one on one therapeutic treatment and care to patients in their home using a comprehensive and patient specific care plan. You will work in tandem with a full team of healthcare professionals including physical therapists, nurses, other therapists and aides. You will have the quality time with the home care patient to promote their health and facilitate independence in their home.

  • Collaborate with the physical therapist in assisting the physician in assessing the patient's functional level by applying appropriate tests. Assist in the development and revision of the Plan of Care

  • Treat patients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy Plan of Care

  • Observe, record and report the patient's response to treatment and any changes in patients condition to the physical therapist and/or director

  • Cooperatively utilize varied solutions to educate and promote the health and independence of the patient which includes instruction in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate

  • Assess, monitor, document and report progress of patient's health and condition using required documents, reports and established timelines

  • Actively participate in patient care conferences to ensure coordination of patient care


  • Current and unencumbered license to practice as a physical therapist assistant specific to the state the therapist is assigned to work by the Company

  • Current CPR certification

  • Valid Driver's License and reliable transportation and agency required liability insurance to perform home visits


  • One (1 ) year of therapy experience with adults, specifically, the geriatric patient population

Summary The OB/GYN Physical Therapy Technician works under the direct supervision of a physical therapist or designated supervisor to help maintain optimal clinical environment. The Technician will assist the physical therapist with patient interaction skills including bringing patients to and from the waiting area, helping to maintain a clean clinical environment, assisting the patient with exercise designated by the physical therapist and helping maintain and update written and illustrated home exercise programs for patients, assisting with application of modalities such as heat and ice to patients, and helping the physical therapist and office staff maintain patient paperwork, records, scheduling, and other administrative support needs. Essential Duties and Responsibilities * Confirms patient identification before bringing them in to the office. * Brings patients in from the waiting area in a timely manner. * Assists the patient in beginning an individualized exercise program as directed by the physical therapist. * Prepares treatment room for patient by following prescribed procedures and protocols which include but are not limited to: preparing beds, gym space and rooms between patients; cleaning equipment, changing out laundry; inventorying and restocking supplies. * Applies heat or ice modalities to regions of the patient's body as directed by the physical therapist * Assists patients in check out process including supporting office staff in scheduling and communicating care plan changes from the physical therapist to the office staff. * Explains procedures to patient to assure understanding and cooperation. * Communicates patient questions and requests with the physical therapist. * Assists patient to treatment tables, gym space or exam rooms and helps monitor for safety. * Updates and organizes patient exercise flow sheets, charts as directed by the physical therapist. * Maintains a safe and clean working environment by complying with procedures, rules, and regulations. * Maintains clinic equipment; completes preventative maintenance requirements; follows manufacturer's instruction; troubleshoots malfunctions if applicable; calls for repairs. * Performs other related work as needed. Other * Maintains other records, reports and files as needed. * Participates in educational activities. * Protects patients by adhering to safety policies and protocols. * Maintains patient confidence and protects operations by keeping information confidential. * Follows organizational policies and procedures, including State of Franklin Healthcare Associates Compliance Manual, Code of Ethics and Business Standards, HR Policies and Procedures Manual, Team Member Handbook, OSHA Manual and any other policies or procedures for the job function, department, location or clinic. * Performs other duties as assigned. Supervises N/A Qualifications Education High School Diploma or GED College Degree in Physical or Exercise Science is preferred, but not required Experience One or more years of work experience related to patient interaction or health and wellness desired.
