Physical Therapist Assistant Home Health

Job Description Job Description Medi Home Health & Hospice , a division of Medical Services of America, Inc., currently seeks an experienced Full-time Physical Therapist Assistant for our Home Health patients in Pulaski (Pulaski, Wythe, Floyd, Giles, Montgomery Caroll counties) VA. · Provides physical therapy services to patients under appropriate guidelines and in accordance with the plan of treatment developed by the PT. · Assists the physician in implementing and conducting treatment as outlined in treatment plan. · Monitor patient's response to treatment and modify treatment as indicated in accordance with the plan of care and reports to the physician or client care manager. · Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies. · Assists with interpreting and communicating evaluation findings to patients, family and other staff members. Job Requirements · Valid PTA license in the state of VA. · Valid Driver's License · Minimum of one-year verifiable physical therapy experience · Home Health experience preferred.
