The Physical Therapist obtains and documents all required and pertinent patient information; orients patients, family and other parties to the purpose and procedures of the Physical Therapy evaluation and treatment; selects and performs evaluations; completes an initial written assessment of referred patients; documents an initial plan of care and completes certification; coordinates treatment schedules; utilizes a team approach in the delivery and management of outpatient services and patient care; selects and provides treatment; identifies significant changes in patient#s condition; provides appropriate patient/family education and teaching; documents the patient#s response to treatment and progress; completes all documentation; updates outpatient plan of treatment and documents; meets requirements of third party payors; completes patient charges; meets productivity standards; demonstrates the ability to adjust own schedule, and monitors program effectiveness and outcomes.

Patients cared for include those in the following age groups:

infant, children, adolescent, adult and geriatric.

Position oversees all human resource functions to include hiring, training, performance reviews and recognition and reward. Develops and champions department performance and behavioral standards and works to continually improve quality of services. Ensures compliance with all regulatory requirements and holds leaders and staff accountable for meeting goals.
