As a steadfast Physical Therapist under our client's wing, your role will entail delivering stellar Physical Therapy services to patients within their homes. Your responsibilities will include evaluating patient functional levels, devising tailored treatment programs, and synergizing with the healthcare team to enhance patient autonomy and physical capabilities.

Superior interpersonal and communication skills and a consistent drive for professional development via continued education and resources are critical for success in this role. Required Qualifications: Applicants must possess a Master's degree or higher alongside a minimum of one year's experience in Physical Medicine & Rehab (PMR) or Sports Medicine.

Our ideal candidate will have a tangible experience with Epic Home Health, or Home Health Care, or home-based health services. To be considered, candidates must have an active and valid Physical Therapist certification/license registered in Illinois (IL), as well as a Regular Driver License (DL). Preferred Qualifications: Please furnish us with your PT license number upon application. Key Responsibilities:- Performing Physical Therapy assessments to gauge patient functional levels and then developing targeted treatment programs.

• Using diagnostic and prognostic tests for muscle, nerve, joint, and functional abilities.

• Treating patients to alleviate pain and restore function.

• Coordinating outpatient services as and when needed.

• Documenting and reporting patient's reactions to treatment and any changes in their condition.

• Guiding patients and their families in using prosthetic, orthotic, and assistive devices.

• Supervising physical therapy assistants and home health aides.

• Preparing regular reports and actively participating in in-service, case conferences, and staff meetings.

• Upholding professional competency through regular in-service attendance and alternative resources.

• Preparing assessments, evaluations, summaries, and clinical/progress notes. Schedule and Shift Details:
  
  The work schedule is from Monday to Friday, 8:00 AM to 5:00 PM with occasional local travel involved.
