Role Description

Phlebotomists are an integral part of Oak Street Health's Clinical teams. High levels of flexibility, attention to detail, initiative and problem solving are required to be successful in this role. You will be expected to build relationships with Oak Street Health patients and team members. Phlebotomists work closely and collaboratively with their teammates and will be expected to assist in assessing patients' health conditions, including performing routine diagnostic testing during appointments.

Responsibilities

  • Perform routine laboratory and diagnostic testing during appointments at our centers

  • Greet and escort patients to and from the laboratory

  • Collect blood and other biological specimen

  • Ensure laboratory orders are properly entered into the Electronic Medical Record (EMR), Greenway and properly linked to lab provider sites (Quest, MACL, Labcorp)

  • Assist patients with signing Advanced Beneficiary Notices

  • Properly confirm orders, label, process, and store specimens for pick-up service by our laboratory partners

  • Adhere to all compliance and regulatory requirements including being a champion of HIPAA compliance and CLIA waived site best practices

  • Train and assist team members for covering the lab in the event of PTO or absence

  • Document patient conditions and visits as appropriate in our EMR

  • Follow all safety, quality, and cleanliness protocols and champion new protocols

  • Coordinate with our laboratory partner to order lab supplies and manage inventory

  • Other duties as assigned by Practice Manager including but not limited to; rooming patients, collecting medical records, assisting patients with scheduling referrals and diagnostic orders

  • Bilingual/ Spanish is required.

Phlebotomists report to the Practice Manager

What we're looking for

We're looking for motivated, experienced phlebotomist with:

  • Certification of Phlebotomy Technician
  • (preferred certifications/ programs: American Certification Agency (ACA) CPT, American Medical Technologists (AMT) RPT, American Society for Clinical Pathology (ASCP) PBT, American Society of Phlebotomy Technicians (ASPT) PC, National Center for Competency Testing (NCCT) NCPT, National Healthcareer Association (NHA) CPT, National Phlebotomy Association (NPA) CPT).

CPR Certification required

  • Demonstrates problem-solving skills, accountability, and a flexible, positive attitude

  • Strong communication skills and customer service orientation

  • Familiarity with Quest Diagnostics procedures and computer-based lab system preferred

  • 2 or more years of experience performing blood draws on geriatric patients

  • EMR experience

  • High level of technological savvy and proficiency in Google suite products

  • Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary)

  • US work authorization

For more information, visit http://www.oakstreethealth.com. Role Description The Behavioral Health Specialist is key resource for Care Teams across a region of Oak Street Health primary clinics. S/he is responsible for coordinating and supporting behavioral healthcare for our patients, in collaboration with Care Teams and a psychiatric consultant. The Behavioral Health Social Worker will assess the needs of patients, make referrals to appropriate behavioral health resources and specialists, and provide short-term counseling and evidence-based treatments as indicated. Responsibilities * Collaborate with the primary care teams to identify appropriate patients to refer into the Oak Street Behavioral Health program * Screen and assess patients for common mental health and substance use disorders * Develop care plans and provide or facilitate referrals to Oak Street's psychiatric consultant and/or external resources * Provide short-term counseling and evidence-based treatments * Coordinate care with Oak Street's psychiatric consultant, including warm hand offs for telehealth visits and collaboration on care plans * Develop relationships with primary care teams and consistently communicate changes in behavioral health care plans * Accurate and timely documentation of patient encounters and counseling sessions in Oak Street's electronic medical record * Participate in regional and organizational efforts to advance Oak Street's Behavioral Health program through workshops, feedback sessions and surveys * Other duties, as assigned What we're looking for We're looking for motivated, experienced licensed clinical social workers with: * A problem-solving orientation and a flexible and positive attitude * Demonstrated ability to collaborate effectively in a team setting * Experience and passion for working with behavioral health conditions * Enthusiasm for working with older adults * Comfort supporting patients with co-occurring behavioral health and substance abuse issues * Experience with screening for common mental health and/or substance use disorders. * Experience with assessment and treatment planning for common mental health and/or substance use disorders * Working knowledge of differential diagnosis of common mental health and/or substance use disorders * Working knowledge of evidence-based psychosocial treatments for common mental health disorders * Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation) * Proficient PC skills * US work authorization * Someone who embodies being "Oaky".
