Description

Purpose of Job

This position is responsible for supporting the systems and software related to Pharmacy technologies and backs up Staff Pharmacist as needed.

Job Functions

  • Implements upgrades and conversions, troubleshoots and reports errors of the Pharmacy Information System.

  • Implements, optimizes, troubleshoots, and reports errors of the medication automated systems.

  • Trains and acts as lead resource for all staff that use pharmacy technologies.

  • Assists with designing, implementing and maintaining software applications relating to medication administration.

  • Assumes role of Staff Pharmacist as needed.

  • Performs other duties as assigned.

Minimum Requirements

Minimum Education/Work Experience/License/Certifications

  • Bachelor's degree in Pharmacy

  • Experience in hospital pharmacy practice

  • Valid Minnesota Pharmacist licensure

Knowledge/Skills/Abilities

  • Capable of making independent decisions with the use of good critical thinking and problem solving skills

  • Knowledge of basic computer functions and Microsoft Office applications

  • Knowledge of IV admixutre, unit doses, oncology, nutritional support and kinetic dosing

  • Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above

Preferred Qualifications

  • Experience with Crystal Reporting and other computer applications

Job Functions * Conducts admission and recertification as ordered by the physician. Completes supporting documentation in an accurate and timely manner. * Coordinates requests/referrals according to patient need and physician order. * Performs supervisory visits and evaluates performance through direct or indirect observation of aides providing care. * Advises and educates patients, family members and significant others on health maintenance/status, disease prevention and/or end-of-life care. * Serves as a patient advocate in seeking appropriate community resources to meet patient need. * Coordinates with and updates the Case Manager in a timely manner. * Completes SOC and Quarterly audits to meet the regulatory requirements as requested by Manager/PCC. * Demonstrates competencies in the specialty area of Home Health. * Performs case management functions under the guidance of a supervisor or primary care clinician as assigned. * Performs other duties as assigned.
